Page 1 of 1 ADDENDUM Addendum No.: 1 Project Name: First Street Sanitary Sewer Bypass Project Project No. 2021-005-28 Date: January 20, 2025 To: All Bidders NOTE: This Addendum forms part of the Contract Documents and modifies the specifications and drawings under Bid No. 2024-03, as noted below. Bidders submitting an offer must sign this form, acknowledging receipt of addendum, and supply it with their proposal. Failure to do so may subject the Bidder to disqualification. CLARIFICATIONS: 1. The asphalt quantity provided in Bid Item 12 includes the asphalt trench cap depth), and the post-milling overlay depth). Any material that is placed as part of the trench cap and subsequently removed as part of the milling operation will not be paid for. 2. All quality control (QC) testing shall be paid for by the Contractor. See the ODOT Field Tested Materials Acceptance Guide for test methods and frequency. Minimum anticipated QC tests include aggregate field-density, asphalt field-density, pipe video inspection, pipe air testing, and manhole vacuum testing. 3. Bid Item 4, “General Excavation”, includes the excavation necessary to construct the Full Depth Reconstruction at the Church Street approach. See sheet 6, note 3.
