Bidders submitting an offer must sign this form, acknowledging receipt of addendum, and supply it with their proposal. Failure to do so may subject the Bidder to disqualification. ITEM 1 PROJECT MANUAL: 1. Replace [EMAIL REDACTED] with [EMAIL REDACTED] 2. Remove sentence on page 4 of Invitation to Bid “Completed pre-qualification forms or proof of pre-qualification shall conform to the Special Provisions.” 3. Replace “Thursday” with “Friday” on Page 6 of Instructions to Bidders. 4. After “Solicitation Schedule” on page 10 of Instructions to Bidders, Add. “Estimate - The estimated range for this project is between $1,100,000 to $1,200,000.” 5. Revise sentence on Page 11 of the Bid Form to read “In the event the Bidder is awarded the Contract and shall fail to complete the work within the 150-day timeframe from the Notice to Proceed, liquidated damages shall be paid to the City of Woodburn, Oregon, of not less than $350.00 per day, until the work shall have been finished, as provided by the Contract Documents.” 6. Add SF (Square Feet) to Units Column, Item 23 on Page 13 of the Bid Form.
