Bid Package for RECYCLING SERVICES CITY OF WHITEFISH - MONTANA ADDENDUM NO. 1 February 25, 2021 ADDENDUM ACKNOWLEDGEMENT Bidders shall acknowledge receipt of this Addendum number and date on the Bid Forms (Pages 10 and 12) of the RFP. Failure to acknowledge this Addendum may be cause for rejection of the bid. The following are amendments to the BID PACKAGE for RECYLING SERVICES, City of Whitefish – Montana. PAGE 4 – PARAGRAPH 3. Clarification to when the Bidder’s Qualification Sheet must be received. Revised Language: The enclosed Bidder's Qualification Statement must be filed with emailed to the Public Works Department not less than five days prior to the opening of the bids by 5 pm on Monday 3/1/2021 and show sufficient financial ability, equipment, and experience to properly perform the contract. The decision of the City as to qualifications shall be final. PAGE 5 – PARAGRAPH 7.A. Additional pages may be submitted with the bid form. Revised Language: A. Bid Forms are included with the bidding documents. These forms must be used, and the Contractor’s bid may not be separated from the attached form. Additional pages may be used to explain and lend clarification to the bid. PAGE 5 – PARAGRAPH 7.C. Clarification to required signatures with the bid and eliminate the requirement of a corporate seal. Revised Language: C. Bids by corporations must be executed in the corporate name by an agent authorized to represent the business entity. the President or Vice President (or other corporate office accompanied by evidence of authority to sign) and the corporate seal must be affixed and attested by the secretary or an assistant secretary. Form 2015A does not exist. Revised Language: F. The bid shall contain an acknowledgment of receipt of all Addenda (the numbers of which must be filled in on Bid Form #1 and Bid Form #2 2015A). PAGE 6 – PARAGRAPH 7.G. The bid does not need to be notarized. Revised Language: G. All bids must be signed before a Notary Public or other officer authorized to administer oaths. PAGE 6 – PARAGRAPH 8 Additional time will be allowed for hard copy submission. Revised Language: Electronic copies of bids shall be submitted to the Whitefish Public Works Department on or before Friday, March 5, 2021, at 4:00 PM according to our clock. Hard copies must be received by Wednesday, March 10, 2021, at 4:00 PM according to our clock. Bids shall be addressed to Craig Workman, Director of Public Works, 418 E. 2nd St., Whitefish, MT 59937, or emailed to [EMAIL REDACTED]. Bids Hard copies shall be firmly sealed in an opaque envelope. Each envelope shall be plainly labeled “Bid for Recycling Services” and shall bear the name of the individual, firm or corporation submitting the bid. If the bid is sent through the mail or other delivery system, the sealed envelope shall be enclosed in a separate envelope with the notation “BID ENCLOSED”. No bid will be considered which is received after the specified closing times.
