Tarbutton Library – Elder Room Renovation Bids will be received by the BOARD OF COMMISSIONERS OF WASHINGTON COUNTY, 119 Jones St, Sandersville, Georgia 31082, until December 13, 2023 by 2:00 PM E.T. The public opening will be on the same day and submissions will be read at 2:00 PM E.T. Bids for the complete work in one general contract shall provide for all materials, equipment, and labor for the construction of: Bid #2024-003 Rosa M. Tarbutton Library – Elder Room Renovation, as specified. All proposals shall be accompanied by a Bid Bond drawn in favor of Washington County, Georgia, in the amount of at least five percent of the lump sum bid for the complete work; such Bid Bond representing that the Bidder, if awarded the contract, will enter into a contract and furnish Performance Bond and Payment Bond as provided by law and approved by the Attorney for Washington County, Georgia. Each bond shall be equal to one hundred percent (100%) of the contract amount. The Bid Bond shall be forfeited to Washington County, Georgia as liquidated damages if the Bidder fails to execute the contract and provide Performance and Payment Bonds within ten (10) days after being notified that he has been awarded the contract. Letters of credit will not be accepted. Bid bonds shall accompany bids. The Owner reserves the right to reject any or all bids and to waive informalities. Any objections to the specifications/contract documents as set forth should be submitted online five days prior to bid openings. Any addenda must be acknowledged prior to award. Such right of action shall be asserted in a proceeding instituted in the name of the Claimant or Claimants for his or their use, and benefit against such Contractor and Surety or either of them (but not later than one year after the final settlement of said Contract) in which action such claim or claims shall be adjudicated and judgment rendered thereon. The Principal and Surety hereby designate and appoint the Owner as the agent of each of them to receive and accept service of process of other pleading issued or filed in any proceeding instituted on this bond and hereby consent that such service shall be the same as personal service on the Contractor and/or Surety. In no event, shall the Surety be liable for a greater sum that the penalty of this bond, or subject to any suit, action or proceeding thereon that is instituted later than one year after final settlement of said Contract. Drawings and Specifications Referenced and attached hereto: Please reference pages noted below in notes specified in the Detailed Scope of Work: • NA Detailed Scope of Work The Contractor shall provide all materials, labor, and equipment and perform all work as described below and per any attached specifications and drawings along with the items below: Demolition • Contractor to provide labor and equipment to demolish the following: o Carpet o VCT in break room o Ceiling cover for projection screen o Acoustical wall treatment o Damaged wood base-Approximately 10' New Work • Contractor to provide labor and equipment to perform the following: o New 2'x 2' carpet tile- color TBD o Paint walls, ceilings, and trim. Color TBD o New LVT in break room o Repair drywall ceiling where projection screen cover is removed. Allowances • Contractor to provide the following allowances: o New shelving in storage closets o New acoustical wall panels Clarification • Contractor to note all pictures and electronics on wall are to be removed prior to start of work. Exclusions • Contractor to note the following: o Painting/refinishing existing doors ---PAGE BREAK--- SCOPE OF WORK Rosa M Tarbutton Library 314 S Harris St. Sandersville, GA 2 Details that Apply to All Work Areas • All work shall be performed to comply with all state and local building codes. • Contractor to maintain clean work areas at all times, remove and dispose of all demolished materials and construction debris. • Parking will be made available for the Contractor by the Owner and the Contractor shall coordinate all parking with the Owner prior to the beginning of work. • Contractor shall coordinate inspections with the Owner as required. • Upon completion of work, all construction areas shall be left clean and free from debris. Warranty • Provide all labor, material, and equipment warranties to the owner prior to project closeout. Submittals • The Contractor shall submit for approval the manufacturer's cut sheets for materials utilized for the project prior to commencing any work. Permitting • Contractor will be responsible for obtaining such applicable permits and the Owner will be responsible for compensating the Contractor for permit fees and any design necessary to obtain such permits or related approvals as described in the EZIQC master contract documents (i.e. permit fees are dollar for dollar reimbursable and professional design and engineering fees are paid for at hourly rates published in the Construction Task Catalog). Owner Responsibilities • Provide access to the worksite during normal business hours. • Provide a staging area for project related materials. • Owner shall be responsible for re-routing pedestrian traffic as necessary. Project Deadline • The Bidder further proposes and agrees hereby to commence the Work with adequate forces and equipment within ten (10) calendar days from receipt of Notice to Proceed, and to complete the work on project by March 31, 2024. If said work is not completed within the time stated, the Contractor shall be liable and hereby agrees to pay the County as liquidated damages and not as a penalty $100 per calendar day for failure to complete the work by March 31, 2024. Liquidated damages will continue until the contract is complete. ---PAGE BREAK--- Contractor Affidavit under O.C.G.A. § 13-10-91(b)(1) By executing this affidavit, the undersigned contractor verifies its compliance with O.C.G.A. § 13-10-91, stating affirmatively that the individual, firm or corporation which is engaged in the physical performance of services on behalf of (Washington County Board of Commissioners)has registered with, is authorized to use and uses the federal work authorization program commonly known as E-Verify, or any subsequent replacement program, in accordance with the applicable provisions and deadlines established in O.C.G.A. § 13-10-91. Furthermore, the undersigned contractor will continue to use the federal work authorization program throughout the contract period and the undersigned contractor will contract for the physical performance of services in satisfaction of such contract only with subcontractors who present an affidavit to the contractor with the information required by O.C.G.A. § 13-10-91(b).
