AFFIDAVIT FOR EXEMPTION FROM JURY DUTY (PRIMARY TEACHER IN A HOME STUDY PROGRAM) GEORGIA, UNION COUNTY JUROR JUROR I hereby affirm that I am a primary teacher in a home study program pursuant to O.C.G.A. 20-2-690(c) and that I have no reasonably available alternative for the home study program and that I request to excused or deferred from jury duty in accordance with O.C.G.A. 15-12-1(a)(1) I am providing to the Court at the time of this request, documentation from the school system indicating that the school system is aware the child is being home schooled and the home school teacher’s schedule for the next six months of classes.
