ID REQUIRED Utah Election Laws now require every voter to provide valid voter identification before being allowed to vote. Please show of the following: ¾ Valid Utah driver license ¾ ID card issued by the state; a branch, department, or agency of the US government ¾ Utah concealed weapon permit ¾ US Passport; or Tribal ID card ¾ Current utility bill ¾ Bank or financial statement ¾ Certified birth certificate ¾ Social Security card ¾ Check issued by the state or federal government ¾ Paycheck from voter’s employer ¾ Current Utah hunting or fishing license ¾ Current US military ID card ¾ Certified naturalization document ¾ Currently valid license issued by an authorized agency of the US ¾ Certified copy of court records showing adoption or name change ¾ Bureau of Indian Affairs card; ¾ Valid Medicaid, Medicare, or Electronic Benefits Transfer Card ¾ Current ID card issued by local government within Utah, an employer, college, or professional school ¾ Currently valid vehicle registration; Two of the following showing name, at least one of them must list current address voter’s residence: If the voter is unable to provide such identification, they will be required to vote a Provisional Ballot and will have five days following Election Day to provide valid voter identification to the County Clerk to make their provisional ballot countable.
