We are thrilled to welcome your child to our Summer Camp! To ensure a smooth and enjoyable experience, please review the following important information to make sure you are prepared for all the fun. What to Wear/Bring: Clothing: Comfortable and weather appropriate as needed. Please label all items with your child’s name. Footwear: Closed-toe shoes for safety during activities. Swim Gear: Swimsuit, towel, and flip-flops for our afternoon camps (please have all in backpack) Water Bottle: A refillable water bottle to stay hydrated throughout the day. Sunscreen: Apply sunscreen before camp and send extra for reapplication. Backpack: To carry personal items and camp supplies. All Day Campers Lunch Box: Please make sure you bring a lunch box or let us know if you would like to participate in the FREE LUNCH AT THE PARK. We will escort campers over to receive the lunch if desired. We do not know what they serve every day, so please send lunch if you have any picky eaters. Snack: Will make time for snacks every hour. Please send snacks if you think your camper would like to snack sometime during camp. Camp T-shirt: Please have them bring to wear for all afternoon activities. All will receive shirts except Baking Camps. You will only receive one shirt per camp season. You may purchase extra for $15. ---PAGE BREAK--- Parking: Due to number of campers registered, we will be doing a drop off/Pick up line instead of parking and coming inside the building. Unfortunately, we only have 15 parking spots (we cannot park in Dental, Human Bean, Police, or Fire Parking lots) so for a fast drop off / pick up we will be forming a parent line. (See pictures on last page) Recreation Building: Morning Camps (9-12pm) Baking Camps (1-4pm) You will form a line in front of the SUMMER CAMP PARENT LINE STARTS HERE SIGN that will start right in front of our Recreation Building. Please line up as far to the right as possible so Police/Fire vehicles can get by. DO NOT BLOCK the FIRE EXITS. Please have your campers unbuckled and ready with all gear in hand for drop off. On Day 1: We will hand you a PICKUP LINE CAMPER NAME to use for quick pick up in line. This will help us get your camper to you in a quicker manner. ONLY those who are on the emergency contact or account may pick up. If you need to have someone else pick up, please email Annie Pew at [EMAIL REDACTED] labeled URGENT PICK UP so I can get that information to our staff. If you are not on the list, we will not release the camper to you. Drop-Off Location: Recreation Building 11665 W. State St. (between Star Fire and Star Police) Drop-Off Time: 9am-9:15am (Half Day/All Day Camps) or 1:00-1:15pm (Baking Camp only) Pick-Up Location: Half Day Campers (9-12pm) Baking Campers (1-4pm) will pick up at Star Recreation Building Pick-Up Time: 12:00-12:15pm (Half Day Camp) or 4:00-4:15pm (Baking Camp) Parking Instructions: Once you have picked up your camper, please swing left through the Human Bean/Dental office out to State St. DO NOT GO BEHIND POLICE AND FIRE BUILDINGS. Do not park in the fire lanes or reserved spaces. ---PAGE BREAK--- Hunter’s Creek Sports Complex: All Day Camper (9-4pm) and Half Day Camp (1-4pm) Half Day Campers Drop Off: (1-4pm) You will form a line in front of the SUMMER CAMP PARENT LINE STARTS HERE SIGN that will start in front of the SUGAR SHACK by the Baseball Fields. Please line up as far to the right as possible so other vehicles can get by. DO NOT BLOCK the FIRE EXITS/Handicap Stalls. Star Road on the South Parking Lot next to the baseball fields/Sugar Shack Drop-Off Time: 1pm-1:15pm (Half Day Campers) (All Day Camps will be bussed over from Recreation Building between 12-12:15pm (Need to have Transportation Form on file to ride in bus) Pick-Up Location: Half Day Campers (1-4pm) All Day Campers (9-4pm) will be picked up at Hunter’s Creek Sports Complex (Next to Sugar Shack and Baseball Fields) Pick-Up Time: 4:00-4:15pm (Half Day Camp/All Day Camp) Parking Instructions: Once you have picked up your camper, please be careful as you exit the park as the kids are playing in the park. ---PAGE BREAK--- Pick-Up Procedures: Authorized Pick-Up: Only individuals listed on the registration form will be allowed to pick up your child. Please bring a valid ID for verification. Late Pick-Up: If you anticipate being late, please notify us as soon as possible. A late fee will apply. Safety is our biggest concern so please make sure your pickup lists are up to date. You will need to sign out your camper every day. You will form that same line for pick up as you did earlier in the day. Lunch & Snacks: Lunch: Please pack a nutritious lunch in an insulated lunch bag with ice packs if necessary. No refrigeration is available. Snacks: Please provide a morning and afternoon snack. If your child has any dietary restrictions or allergies, please inform us in advance. Allergy Awareness: Please make sure if your camper has any allergies, they are listed on the sheet or notify Annie Pew at [EMAIL REDACTED] WEST ADA DOES HAVE THE SUMMER LUNCH PROGRAM AVAILABLE AT HUNTER’S CREEK FROM 11:45-1PM for those interested. For my ALL-DAY CAMPERS, we can take them over there to get the free lunch. Please let us know in drop off that they will need the free lunch! Our Summer Camp STAFF: Our staff spend weeks training in behavior management, Safety/First Aid/CPR training, Program planning, communication skill building, Teamwork/Leadership collaboration, and camp policy and procedures. All our staff are CPR certified and all adults have had a background check. We are confident that this training has equipped our teen staff with the skills and knowledge necessary to provide a safe, fun, and nurturing environment for all campers. I understand that transportation will be provided by City of Star Recreation and agree for the camper to be transported by City of Star or by transportation arranged by City of Star. I understand that attendance is voluntary. I represent and acknowledge that I am the parent or legal guardian of the camper indicated above and am fully competent to sign this Agreement. I acknowledge that I am signing this Release, Waiver of Liability and Hold Harmless Agreement on behalf of myself and the above-mentioned camper, and I agree to be bound by the terms of this Release, Waiver of Liability and Hold Harmless Agreement. Assumption of Risk: I acknowledge that the mode of transportation and nature of the summer camp/activity may expose camper to hazards or risks that may result in illness, personal injury, or death and I understand, and I accept all risk to my health that may result. I recognize and acknowledge that certain risks of harm are or may be inherent in the various activities contemplated herein and that the City of Star cannot control all of these risks. I acknowledge there may be physically strenuous activities and certify by my signature that the camper is physically fit and able to participate. I agree that the camper will abide by all City of Star rules and guidelines set forth for the summer camp or activity. Box 130 Star, Idaho 83669 [PHONE REDACTED] [EMAIL REDACTED] Release: In consideration of City of Star transporting the above-referenced camper and permitting camper to participate in the summer camp/activity identified above, I, individually and on behalf of the above referenced camper, release City of Star employees and representatives from any and all liability to me, individually and on behalf of the camper, my personal representatives, and assigns for any and all claims, demands and causes of action for any and all loss of personal property, illness or injury, including death, arising out of, resulting from, caused by, occurring during or in any way connected with the summer camp/activity, including injuries caused by negligence of City of Star and/or it’s representatives, or any other participant in the summer camp/activity that may be sustained by me, individually or on behalf of the camper, while participating in such summer camp/activity, or while on premises owned or leased by City of Star. Personal Insurance: I understand that City of Star does not provide medical insurance for the camper. I am urged by City of Star to maintain adequate personal health and accident insurance to cover any personal injury to the camper which may be sustained while participating in this summer camp or activity. Indemnity, Hold Harmless, and Waiver: I agree to indemnify and hold harmless, waive and covenant not to sue City of Star, it’s representatives from liability for the injury or death of any person(s) and damage to property that may result from the camper’s negligent or intentional act or omission while participating in the described summer camp/activity. It is my express intent that this Covenant Not to Sue and Agreement to Hold Harmless shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representatives, if I am deceased, and shall be governed by the laws of the State of Idaho. I HAVE CAREFULLY READ THIS AGREEMENT AND UNDERSTAND THAT IT IS A RELEASE, WAIVER AND HOLD HARMLESS OF LIABILITY OF ALL CLAIMS AND CAUSES OF ACTION FOR ANY INJURY OR DEATH TO MYSELF, INDIVIDUALLY OR ON BEHALF OF THE CAMPER, THAT OCCURS WHILE PARTICIPATING IN THE DESCRIBED SUMMER CAMP OR ACTIVITY, AND IT OBLIGATES ME TO INDEMNIFY THE PARTIES NAMED FOR ANY LIABILITY FOR INJURY OR DEATH OF ANY PERSON AND DAMAGE TO PROPERTY CAUSED BY CAMPER’S NEGLIGENT OR INTENTIONAL ACT OR OMISSION.
