TOWN/VILLAGE COURT of CONDITIONAL DISCHARGE STATE OF NEW YORK: COUNTY OF SCHUYLER PENAL LAW § 65.05 & 65.10 PEOPLE OF THE STATE OF NEW YORK , -against- Plaintiffs, ORDER AND CONDITIONS OF CONDITIONAL DISCHARGE Defendant. On the day of you were convicted in this Court of This Court places the Defendant on a Conditional Discharge for a period of year(s) from the date of this Order. While under supervision, you shall observe the following conditions of supervision. THE COURT IMPOSES THE FOLLOWING CONDITIONS; THE DEFENDANT: 1. Must report to the Court when directed by the Court 2. Must remain within the jurisdiction of the Court unless granted permission to leave by the Court. 3. Must notify the Court of any change in address or employment. 4. Must work faithfully at suitable employment, or pursue a course of study to gain employment. 5. Must obey all laws of New York State, the United States of America and any municipal division. 6. Must ovoid injurious or vicious habits, including the use of un-prescribed or illicit drugs. 7. Must cooperate and participate in an evaluation and counseling program, including Mental Health and/or drug and alcohol treatment 8. Must pay restitution in the amount of (Please include a restitution order) 9. Must pay a fine in the amount of to the sentencing court. The period of Conditional Discharge shall expire on terminated by the Court prior to the aforementioned date.
