City of Puyallup ADOPT-A-TRAIL PROGRAM ADOPT-A-TRAIL VOLUNTEER LIABILITY WAIVER FORM PLEASE READ LANGUAGE ON PAGE 1 BEFORE SIGNING VOLUNTEER LIABILITY WAIVER FORM WARNING - PLEASE READ CAREFULLY Participants in the Puyallup Parks and Recreation Adopt-A-Trail Program (“Program”) are advised that working on city trails is an activity with associated risks of physical injury and hazards. Each participant is further advised that the risk of harm is present even if all safety precautions are followed. Each participant shall exercise due care in performing assigned duties as an adopt-a-trail volunteer. Adopt-a-trail volunteers must receive safety training provided by the City of Puyallup utilizing materials and training aids furnished by the City prior to participating in any trail volunteer related activities. PARTICIPANTS ARE AGAIN ADVISED THAT THE SAFETY PRECAUTIONS DESCRIBED ABOVE DO NOT GUARANTEE THEIR SAFETY WHILE A PARTICIPANT IN THE PROGRAM. ASSUMPTION OF RISK AND HOLD HARMLESS. By choosing to participate in the Program, I assume all risks associated with my participation in the Program. On behalf of myself, my heirs, and assigns, I hold the City of Puyallup, its officers, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney’s fees, arising out of in connection with my participation in the Adopt-A-Trail Program, except for injuries and damages caused by the sole negligence of the City.
