This application is primarily to ensure the tree is located properly and the species to be planted in city right-of-way is acceptable. When preparing this application, please print or type the reply to each question. Call 811 at least 2 days before you dig to have the water, electric, and gas lines marked on your planting strip. Are overhead utilities present? If so, plant a tree that reaches a mature height of 25 feet or less. Select the “Right Tree for the Right Place.” Consider mature size, root damage potential, allergy problems, and insect and disease susceptibility. Is your tree on the City of Puyallup’s approved street tree list? Trees must be planted to these minimum standards: 2½ feet back from the face of the curb 5 feet from underground utility lines 10-15 feet from utility poles, driveways, alleys, hydrants 30 feet from street intersections.
