City of Puyallup ADOPT-A-TRAIL PROGRAM APPLICATION FORM - ADOPT-A-TRAIL PROGRAM Thank you for your interest in the Adopt-A-Trail program. Adopt-A-Trail volunteers promote community stewardship and bridge the communication between the community and the city. The Adopt-A-Trail program sends a message that the community cares about its trails. Through visits to your trail, volunteers document maintenance needs, report acts of vandalism, interact with trail users, promote trail safety and encourage an overall positive use of public trails. This position is a minimum one-year commitment. Potential adopt-a-trail steward applicants will not be recognized under the program until filing out this application and discussing the program details at a ‘walk- and-talk’ trail orientation. Some are multi-use, paved trails, while others are more rugged, hiking or nature- oriented trails. Please provide us any additional information that you believe is pertinent to your request to become an adopt-a-trail volunteer ---PAGE BREAK--- VOLUNTEER LIABILITY WAIVER WARNING - PLEASE READ CAREFULLY Participants in the Puyallup Parks and Recreation Adopt-A-Trail Program (“Program”) are advised that working on city trails is an activity with associated risks of physical injury and hazards. Each participant is further advised that the risk of harm is present even if all safety precautions are followed. Each participant shall exercise due care in performing assigned duties as an adopt-a-trail volunteer. Adopt-a-trail volunteers must receive safety training provided by the City of Puyallup utilizing materials and training aids furnished by the City of Puyallup prior to participating in any trail volunteer related activities. All volunteers will coordinate any physical work with the program coordinator prior to work occurring to ensure safety and rules regarding liability are followed. By signing this application, I agree to notify the program coordinator of any planned maintenance activities and will ensure all volunteers sign the group volunteer waiver form provided on the adopt a trail webpage. PARTICIPANTS ARE AGAIN ADVISED THAT THE SAFETY PRECAUTIONS DESCRIBED ABOVE DO NOT GUARANTEE THEIR SAFETY WHILE A PARTICIPANT IN THE PROGRAM. ASSUMPTION OF RISK AND HOLD HARMLESS. By choosing to participate in the Program, I assume all risks associated with my participation in the Program. On behalf of myself, my heirs, and assigns, I hold the City of Puyallup, its officers, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney’s fees, arising out of in connection with my participation in the Adopt-A-Trail Program, except for injuries and damages caused by the sole negligence of the City.
