Current proof of income (within the past 30 days) for each member in household: Work check stubs Notice of Action or Verification of Benefits Last two unemployment payment stubs or current readout of benefits Social Security/Disability statement of benefit letter You must show your benefits effective January of the current year Previous year’s tax return Income Certification from SAVES 2. Proof of address only ONE of the following needed: Current Gas or Electric Bill in adult’s name living in household Rental/lease agreement 3. Proof of ALL adults in household: Valid Picture and Social Security Card Valid Passport 4. Proof of ALL minors in household (only one of the following needed for each child): Birth Certificate Picture School ID Medi-Cal Card Valid Passport IMPORTANT NOTICE FOOD ASSISTANCE IS AVAILABLE A MAXIMUM OF FOUR TIMES IN A FISCAL YEAR (JULY 1-JUNE 30) WITH PROOF OF AN EXTRA EXPENSE, AND IT MUST BE 30 DAYS SINCE YOUR LAST FOOD SERVICE. FOR YOUR SECOND, THIRD AND FOURTH FOOD ORDER FOR THE YEAR, YOU WILL NEED AN EXTRA EXPENSE RECEIPT OF $25 DOLLARS OR MORE. For example: • Car repairs, car registration, moving cost/deposits, prescriptions/medications, medical payments, court costs or utility bill deposits. • The extra expense must be for the month you are requesting services. Your expense must be of an amount that would prevent you from purchasing groceries. Utilities, entertainment or any other regular expense is not considered an extra expense. Comprobante de domicilio. Solo necesita uno de los siguientes comprobantes: Factura actual de gas o electricidad en el nombre de uno de los adultos que viven en el hogar Contrato de arrendamiento 3. Prueba de todos los adultos en el hogar: Identificación validad y Seguro Social Pasaporte Valido 4. Prueba de todos los menores de edad en el hogar (solo necesita uno de los siguientes comprobantes por niño): Acta de Nacimiento Identificación de la escuela Tarjeta de Medi-Cal Pasaporte Valido AVISO IMPORTANTE UNO MÁXIMO DE CUATRO DESPENSAS DE COMIDA EN UN AÑO FISCAL (1 DE JULIO AL 30 DE JUNIO). ENTRE CADA DESPENSA TIENE QUE HABER UN LAPSO DE 30 DÍAS. PARA SU SEGUNDA, TERCERA Y CUARTA DESPENSA DEL AÑO USTED NECESITA UN RECIBO DE GASTOS ADICIONALES DE $25 DÓLARES O MÁS. Ejemplos: • Reparaciones de vehículos, matriculación de vehículos, gasto de mudanza/depósitos de arrendamiento, medicamentos, gastos médicos, costos de corte o depósitos en cuentas de servicios públicos. • Los gastos adicionales debe ser para el mes que usted está solicitando servicios. Su gasto debe prevenir que usted compra alimentos pore se mes. Servicios públicos, de recreacion o cualquier otro gasto mensual regular no se considera un gasto adicional.
