Amendment: All references to insurance requirements, including, Appendix D-1, is included in Section 17 of the Appendix B, Sample Agreement. 2. Amendment: Appendix D-2, Certified Evidence of Insurance, is located below and shall be incorporated into the RFQ. I further understand that prior to execution of agreement, if as the lowest responsive Consultant I fail to comply strictly with the insurance requirements, I will be found non-responsive and the City of Lancaster will turn to the next lowest responsive Consultant.
