NOTICE: SOCIAL SECURITY ADMINISTRATION Due to staffing and safety concerns, the Social Security Administration is no longer providing outreach to any location on a regular basis. Please remind the public that SSA continues to offer online and phone services to the public. You can also refer the public to www.socialsecurity.gov, where they can create a my Social Security account for the following services: • new Get personalized retirement benefit estimates using the new Retirement Calculator • new Opt out of mailed notices for those available online • Check your application status • Set up or change direct deposit • Request a replacement Social Security card (unavailable in Nevada) • new Access the Representative Payee Portal • Get a Social Security 1099 (SSA-1099) form • Get a proof of income letter • Change your address if you’re a beneficiary Online submission for replacement social security cards is currently unavailable in Nevada, therefore only original documents should be mailed to the local office at 1250 S. Buffalo Dr., Suite 150, Las Vegas, NV 89117-8329. The application for the replacement cards and most Social Security forms and publications can be printed from our website. People can also find their local field office phone and fax number by accessing the Field Office Locator.
