Amended: November 3, 2025 Approved by the Board of Commissioners: November 12, 2025 Otsego County Animal Control Shelter 256 Fairview, Gaylord, MI 49735 TODAY’S (989) 705-7632 COMPANION ANIMAL FOSTER/TRIAL AGREEMENT FOSTER______ TRIAL______ PRINT PHYSICAL ADDRESS MAILING ADDRESS HOME CELL WORK BY ACCEPTING THE ANIMAL FROM OTSEGO COUNTY ANIMAL CONTROL SHELTER, I AGREE TO THE FOLLOWING: o To keep the below described animal in my care while on trial/foster o If at any time the above animal needs medical attention during trial/foster period, I agree to return the animal to the Otsego County Animal Control Shelter (OCACS) for treatment. o I will be held responsible for all medical bills accrued if I take the below described animal to any veterinarian without the consent of the Otsego County Animal Control Shelter during the foster/trial period. o Bringing an animal into your home is a lifetime commitment and the trial period is designed to help determine if you would like to permanently adopt the animal. If you have problems or concerns contact us immediately or return the animal to the OCACS before the adoption is finalized on the date below. o OCACS reserves the right to demand the return of an animal adopted from our facility when there is proof the animal (under federal, state or local law) is being neglected, abused or otherwise not being cared for (i.e. habitually running at large, repeated nuisance complaints) o While the OCACS makes every effort to place only healthy animals, it cannot guarantee the health of any animal adopted from the OCACS, or be responsible for any medical expenses which may be incurred.
