20TH STREET WIDENING PREQUALIFICATION – PAGE 1 REQUEST FOR QUALIFICATIONS NOTICE: Ogden City ("Owner") is seeking Statements of Qualifications (SOQs) from qualified General contractors for the 20TH STREET WIDENING PROJECT: The project will widen approximately 0.75 miles of 20th Street, between Washington Boulevard and Quincy Avenue, in Ogden Utah by approximately 12 feet. Due to the roadway widening along 20th Street, certain intersecting side streets will be reconstructed as to tie into 20th Street appropriately. Additionally, approximately 65 residential and commercial driveways will be reconstructed. Approximately 42,000 square feet of concrete sidewalk, and 8,600 linear feet of curb and gutter will be constructed as part of the widening. Project wide, approximately 4,500 feet of storm drain line will be constructed. Additionally, 5,500 feet of culinary water line and 2,100 feet of sanitary sewer will be constructed or reconstructed. The project will also install new traffic signals at the Monroe Boulevard intersection, a pedestrian crossing signal at the Madison Avenue intersection, and a Traffic Signal at the South East corner of the Washington Boulevard intersection. REQUEST FOR QUALIFICATIONS (RFQ): Interested firms may obtain the RFQ Information Packets at no charge by downloading on the Ogden City Website, SOQs DUE: Statements of Qualifications will be received at the 2nd Floor Information Desk of the Ogden Municipal Building located at 2549 Washington Blvd, Ogden Utah 84401 no later than 4:30 PM on Friday, January 17, 2020. Six copies of the contractor's SOQ shall be submitted in a sealed envelope. Any SOQ failing to clearly present all the requested information or failing to be in the requested format may be considered non-responsive and rejected. Late submittals will not be accepted. SELECTION OF QUALIFIED FIRMS: The Owner will review the submitted SOQs for required content, expertise, and experience. The Owner will select the firms to be pre-qualified. NOTIFICATION OF QUALIFIED FIRMS: All contractors submitting pre-qualification proposals will be notified via the city's website by Friday, January 24, 2020 whether they did or did not pre-qualify. Only pre-qualified contractors will be invited to bid on the project. The Owner's decision will be final. The Owner reserves the right to reject any and all pre- qualification proposals. Pursuant to the Utah Government Records Access and Management Act (GRAMA), records will be considered public after the contract is awarded. If an offeror wishes to protect any records, a request for business confidentiality may be submitted to the Ogden City Recorders Office at the time of bid submission. Published: December 29, 2019 & January 5, 2020 ---PAGE BREAK--- 20TH STREET WIDENING PREQUALIFICATION – PAGE 2 REQUEST FOR STATEMENT OF PRE-QUALIFICATIONS FOR 20th Street Widening Project (Washington Boulevard to Quincy Avenue) Prepared for: Ogden City January 2020 CONTRACTOR PREQUALIFICATION REQUIREMENTS 20th Street Widening Project (Washington Boulevard to Quincy Avenue) Ogden City (Owner) requests general contractor prequalification proposals for construction of the 20th Street Widening Project (Washington Boulevard to Quincy Avenue) and this document describes the requirements for their prequalification. The submittal requirements for prequalification purposes and the evaluation criteria are as defined herein. PURPOSE The purpose of the prequalification process is to select those general contractors the Owner deems to be qualified and capable of completing the Project on schedule in conformance with the Contract Documents. Only those firms that are prequalified by the Owner will be allowed to submit a Bid on the Project. PROJECT DESCRIPTION The Project consists of the following: • Widen, and rebuild approximately 0.75 miles of 20th Street, between Washington Boulevard and Quincy Avenue. The street will have an average of 12 feet of asphalt added to the existing roadway. • Install new traffic signal at the Monroe Boulevard intersection and one signal pole at the Washington Boulevard intersection. Install pedestrian crossing signal at Madison Avenue intersection. Remove and replace street lights along the corridor and reconnect to new poles on the project. • Due to the roadway widening along 20th Street, certain intersecting side streets will be reconstructed as to tie into 20th Street appropriately. • Approximately 65 residential and commercial driveways will be reconstructed. • Approximately 42,000 square feet of concrete sidewalk, 8,600 linear feet of curb and gutter, along with a new asphalt roadway will be constructed as part of the widening. • Storm drainage system improvements including approximately 4,500 feet of storm drain main line will be constructed. • 5,500 feet of culinary water line will be constructed or reconstructed. ---PAGE BREAK--- 20TH STREET WIDENING PREQUALIFICATION – PAGE 3 • 2,100 feet of sanitary sewer mainline and 18 sewer manholes will be constructed or reconstructed. • Site grading and drainage, erosion control, piping, retaining, and fencing. • Landscaping as indicated on the plans in the disturbed areas. SUBMITTAL PROCEDURE Prequalification proposals will be received at the 2nd Floor Information Desk of the Ogden Municipal Building located at 2549 Washington Blvd, Ogden Utah 84401 no later than 4:30 PM on Friday, January 17, 2020. Six copies of the general contractor’s proposals shall be submitted in a sealed envelope. Any proposal failing to clearly present all the requested information or failing to be in the requested format may be considered non-responsive and rejected. Contractors will be notified of prequalification by Friday, January 24, 2020. Project Drawings and Specifications will be available on Wednesday, January 29, 2020 to prequalified bidders. Information contained in the prequalification proposals will be considered confidential and reasonable precautions will be taken to ensure the security of the documents. All proposals will become and remain the property of the Owner. The Owner reserves the right to request a contractor to clarify any part of his proposal. Response to such requests must be made in writing and will become part of the proposal. Unsolicited supplementary information and materials received after the deadline will not be considered in the evaluation. TENTATIVE PROJECT SCHEDULE The tentative project schedule is as follows: • Prequalification proposals due Friday, January 17, 2020 • Prequalified contractors announced Friday, January 24, 2020 • Contract documents available to prequalified contractors Wednesday, January 29, 2020 • PreBid meeting Wednesday, February 5, 2020 • Bid opening Thursday, February 20, 2020 • Substantial completion Monday, October 26, 2020 • Final completion Wednesday, November 25, 2020 This schedule is preliminary and is subject to change. CONTENT OF PROPOSAL The general contractor prequalification proposal shall include the information requested below. All information shall be bound with indexes separating the sections. The submitted proposal shall be no longer than 10 pages. Double sided will be counted as two pages. The requested forms and schedule will not count towards the 10 page maximum requirement but must be included as part of an appendix for the proposal. Section 1 B Prequalification Information Form The contractor shall complete and sign the attached General Contractor Prequalification Information Form and shall supply the additional support information requested for Items 14, 15, 16 and 17 of the Form. Section 2 B Key Personnel and Qualifications Provide information that will identify key personnel who will be assigned to the Project, including project manager, project superintendent, and project scheduler. Provide detailed resumes for these individuals. Section 3 B Work Approach and Scheduling Describe your approach to scheduling and accomplishing the work to meet the Project schedule. Section 4 B Previous Project Experience A. List all projects of $1 million construction cost or larger accomplished in the past five years similar to this type of project. • Name of project • Description of project • Owner • Engineer • Year completed • Dollar value of work performed • Estimated time to complete the project versus actual time to complete the project. Provide the following information: • Experience resume of the person who will be designated chief construction superintendent • A notarized and verified financial statement, references, and other information, sufficiently comprehensive to permit an appraisal of contractor’s current financial condition. • Identify which portions of the listed components of the work, you as general contractor would perform with your own crews and which work will be completed using a sub-contractor. Please also include the anticipated sub-contractor who will perform the work: Demolition (including structures like retaining walls) Excavation and backfill Vegetation Removal including trees and private landscaping Concrete and forming work (including slip form work for curb and gutter) Asphalt for roadway construction Traffic Signals Electrical work for City Maintained facilities (Street lights, School Reduced Speed Zone Flashers, etc.) Storm Drain main/facilities Culinary Water Main Installation/reconstruction/appurtenances Sewer Lateral Recommissioning Site grading and drainage Revegetation Section 5 B Financial Information Submit a certified financial statement for the contractor’s most recent accounting period. Submit a letter from the contractor’s surety company specifying the contractor’s total bonding capacity and current unused bonding capacity. Submit additional references and information sufficiently comprehensive to permit an appraisal of the contractor’s current financial status. The Owner shall be entitled to contact each reference listed by the contractor. The contractor, by submitting a prequalification proposal, expressly agrees that any information concerning the contractor in possession of said entities and references may be made available to the Owner. Information requested, but not supplied by said entities and references may be grounds for adversely considering a contractor’s prequalification application. EVALUATION OF PROPOSALS Prequalification of prospective general contractors will be determined by the application of a pre- established pass/fail rating system to the following information submitted by applicants. • License • Completion of work by surety ---PAGE BREAK--- 20TH STREET WIDENING PREQUALIFICATION – PAGE 6 • Bonding capacity • Key staff experience • Project experience • Schedule performance • Qualification of proposed subcontractors To be qualified a general contractor must meet the following minimum criteria: • Hold a current Utah General Engineering Contractor License. • Have no project completed by a surety in the past five years. • Have $7 million minimum available bonding capacity. • Completed two similar road project within the last 10 years. These projects must be listed as two of the projects from the Contractor’s Prequalification Form. • Have significant experience (minimum ten (10) years) in the construction road widening/reconstruction projects with associated underground utility mainline work. • Have a project manager with a minimum of five years of experience as a project manager on at least two roadway widening projects. These projects must show a minimum of 1,000 linear feet of installation of a utility mainline (culinary water main, sanitary sewer main, or storm drain main lines). Utilities cannot be combined to make 1,000 linear feet (e.g. 500 linear feet of culinary water main and 500 linear feet of sanitary sewer main does not make 1,000 linear feet for a project to count). Specifically, the project manager shall have served on one of the projects which is listed in the general contractor’s Prequalification Information Form. • Have an experienced construction superintendent with at least ten (10) years of experience as a construction superintendent, serving as superintendent on at least two roadway widening projects. These projects must show a minimum of 1,000 linear feet of installation of a utility mainline (culinary water main, sanitary sewer main, or storm drain main). Utilities cannot be combined to make 1,000 linear feet (e.g. 500 linear feet of culinary water main and 500 linear feet of sanitary sewer main does not make 1,000 linear feet for a project to count). One project must be listed in the general contractor’s Prequalification Information Form. • Have a project scheduler with a minimum of three years of experience in roadway widening projects in conjunction with underground utility work. • Provide an example CPM schedule on one of the projects defined in the general contractor’s Prequalification Information Form which was completed by the scheduler. • Have completed two (or more) underground utility projects for culinary water/sanitary sewer/storm drain in excess of 1,500 linear feet. These projects must be listed in the contractor qualifications form. • Have experience including installation of electrical facilities including traffic signal system and LED Street Lighting on existing power poles. The evaluation is solely for the purpose of determining which general contractors (together with their proposed subcontractors) are deemed qualified for successful on-time performance of the type of work included in this Project.
