December 7, 1998 CHANGE ORDER NO. 2 Project: Wastewater Treatment Facilities Headworks 1. Modify the grit classifier piping size. The new pipe and fittings were increased in size from the 3" that was shown on the original plans to 4" to allow better hydraulic conditions for the existing pumping equipment. I Add $630.00 2. Construct Kevstone Retaining Wall. A retaining wall was constructed around the segment of the existing primary clarifier nearest the new building. This allowed the drive approach in front of the garage door of the new building to be constructed at a more reasonable gradient. The wall was not included in the original scope of work. Add $2,207.00/ 3. Change gas line trench backfill type. The Washington Water Power requested sand in lieu of native material to bed their gas line that was installed to serve the new building/ Add $317.00 4. Delete landscaping repair. The contractor was required (per the contract) to return all damaged landscaped areas to pre-construction condition at the end of the project. A decision by the City to extend the limits of site paving after the contract had been awarded deleted the need for some landscape restoration work. Deduct $700.00,... 5. Yard piping modifications. a. A temporary 24" HOPE bypass line installed as per Change Order No. 1 was left in place for possible future use - in lieu of removing it as had been originally planned - a credit was given (-$565.00): b. One new storm manhole was decided to be eliminated from the scope of work - a credit was given ( -$645. 00): c. Extra work was required to remove an abandoned buried concrete structure that was discovered when trenching to install a new storm line (+$290.00). Deduct (a+b+c) $920.00 6. Miscellaneous electrical additions: a. Replace conduit and wire to existing garage storage; b. Wire mag for chlorine pump; c. Power control for grit pump; d. Extra Di points; e. Two level sensors; f. Water temperature sensors.
