ADDENDUM TO PROFESSIONAL SERVICES AGREEMENT THIS ADDENDUM TO PROFESSIONAL SERVICES AGREEMENT made this £11 day of A/OIIf.d/!fik. 2012, between the City of Moscow, Idaho, a municipal corporation of the State of Idaho, 206 East Third Street, Moscow, Idaho, 83843 (hereinafter "CITY"), and Class Act Janitorial, Inc., 13601 McMillan, #102-223, Boise, Idaho, 83713 (hereinafter "JANITOR"); WITNESS E TH: WHEREAS, CITY is responsible for maintenance and operation, including janitorial services, for all or part of the Intermodal Transit Center (hereinafter "ITC") located at 1006 Railroad Street in Moscow, Idaho; and WHEREAS, CITY currently has a Professional Services Agreement with JANITOR to perform work in various CITY -owned and/or controlled buildings; and WHEREAS, the term of the Professional Services Agreement continues through September 11, 2014; and WHEREAS, CITY and JANITOR wish to add the ITC janitorial services/responsibilities to the current Professional Services Agreement scope per attached Exhibit B; and WHEREAS, JANITOR has agreed with CITY to· provide janitorial services at the same standard and quality for the ITC as it is obligated to provide pursuant to the Professional Services Agreement; and 1 • WHEREAS, CITY and JANITQ.R have agreed that JANITOR shall provide ITC janitorial services for an amount riot to exceed one thousand five hundred twenty nine and fifty one/hundredths dollars ($1,529.50) per month through the term of the existing Professional Services Agreement; NOW, THEREFORE, it is agreed as follows: Pursuant to the Professional Services Agreement entered into between CITY and JANITOR on the 8th day of September, 2011 (also known as Moscow City Contract 2011-45), paragraph N. Modification; JANITOR shall provide janitorial services to CITY for CITY's Intermodal Transit Center (ITC) through the term of the existing Agreement ending September 11, 2014 for the rate not to exceed one thousand five hundred twenty nine and fifty one/hundredths dollars ($1,529.50). Daily Service shall be performed in conformance with the following schedule: a. 5 days per week (Mon-Fri) 2. JANITOR shall perform the following Daily Service: a. Empty all waste baskets and place contents in dumpster provided outside of building b. Empty, sanitize, and re-line interior of sanitary containers. c. Sweep and/or dust mop and/or vacuum all floor surfaces. d. Dust cleared desks, chairs, tables and other office surfaces. e. Dust counters, file cabinets, all ledges and other surfaces within reach. f. Wet mop restrooms and lobby area. g. Clean and sanitize sinks, toilets, urinals and other restroom fixtures, including mirrors. h. Wipe down stall walls in restrooms. 1. Clean and refill all restroom dispensers. J. Wash entrance doors and entrance glass (interior and exterior). k. Clean and sanitize drinking fountains. 1. Vacuum all carpets. m. Keep JANITOR closet clean and orderly. n. Wipe clean all counters and tables in break rooms and conference room. 3. JANITOR shall perform the following Weekly Service: a. Damp mop all vinyl and tile floors, including break rooms. b. Remove scuff marks as necessary. c. Clean interior glass within reach. d. Clean cleared desk tops, counter tops, file cabinets, wastebaskets, drawer faces. e. Clean light fixture exteriors. f. Wash and clean restroom walls and kitchen walls near sinks, trash cans and ranges. h. Clean stains or spots on carpets. 1. Buff all tile, wood and terrazzo floors to maintain a high shine. J. Vacuum all upholstered furniture. 4. JANITOR shall perform the following Service: a. Dust all areas and clean interior glass requiring a ladder to reach. b. Wash and clean all walls. 5. JANITOR shall perform the following quarterly service four times per year (October, January, April, and July): a. Professionally clean all carpets and upholstered furniture. b. Scrub and wax all tile, vinyl and terrazzo floors. 6. JANITOR shall disassemble and clean insects from ceiling light fixtures three times per year (June, September, and October). 9. We thank you for the opportunity to present a bid for our services. We are looking forward to meeting your janitorial needs. We carry comprehensive liability insurance and janitorial fidelity bond. Our rate include all costs materials equipment supervision wage taxes and insurance that is required to fulfill your janitorial need. Does not include paper products, trash liners or soap.
