This listing to be included with AlA Document G701, Change Order. Additional documentation may be included. Change Proposal #5 Items- Item 1 Item 2 Delete the painting of the primary structure inside of the building (beams and columns). Exterior columns and beams still to be painted. See change proposal drawing for areas where the primary steel is to remain painted. Deduct $3,500.00 Add the following changes as requested by Latah Sanitation on 1/31/94 and 2/01/94: Electrical- 1. Redemption Counter- Power and two circuits are to be added to this location. The power is to be fed from overhead by conduit which will come down the exterior wall then extend over to a location above the counter. This conduit will need to be supported from the roof structure above. 2. Horizontal Baler- This unit is to be located next to the glass crushers on the north wall. 3. Glass Crushers- The glass crushers are to be turned 90 degrees so they face south. 4. Can Blower and Crusher- This unit needs more power than was originally indicated (See the enclosed letter from Doyle Engineering). Both items according to Strom Electric require 230 V, 3 phase connections with a 60 amp circuit. Additionally a 120 V outlet is required for a scale to be located in this area. Conference Room- The west wall of the conference wall is to have an additional receptacle added along with a telephone jack. The east wall is to receive an additional receptacle. Cabinets- 1. Modification of Cashier counter tops- Lower unit at northeast corner to be omitted with counter for knee space in this area. Lower counter top at windows to be narrower in width. Note: This work has already been done. Provide user with stock items including extra wall panel material. 2 • Clean the facility and site in general. 3 • Demonstrate the operation and maintenance of the equipment (including mechanical, electrical and scale equipment) to personnel designated by the owner with Latah Sanitation. 4 • Balance systems according to the specifications. 5 • Provide 0 & M manuals per specifications including warranties. 6 • Provide record documents per specifications. Including as built drawings, record project manual and record submittals. 7. Follow substantial completion and final completion procedures as outlined in Section 01025, Payment, Modification, and Completion Procedures. 8 • Switch over meter readings to the Owner. 9. Verify all keying is appropriate and works properly. Cashier's Office- ! . Provide State certification of the scale. 2 • Cover for cable from scale pit. 3 . Outlet cover for scale unit. 4 . Missing vinyl wall covering at sink waste drain (at escutcheon). 3 • Clean Room. 4 • Finish closet cabinet at receptacle. 5. Water heater relief drain needs to be connected. 6 • Check toilet fixtures and toilet accessories work. 7. Fan does not turn on. Men's Rest Room- 1 • Remove duct tape at wall base. 2 . Clean room. 3. Transition at V.T. and sheet vinyl. 4 . Fan does not turn on. 5 • Wall base needs to be applied where accessible. Conference Room- 1. 45 degree wall base corners. 2 . Add rubber base. 2 • Remove masking tape from girt next to large overhead door (east of door). 3 . Stripping and text needs to be painted on floor. Coordinate this with equipment installation by end user (Latah Sanitation). 4 • Tighten nut at column base near south wall disconnect of can blower. 5 • Paint man doors and east roll up. 6 • Touch up major areas of rust on wall girts with primer (especially bad on the north wall). 7 • Verify wall dampers and roof vent units work properly. 8. Fluorescent lights over main entry to point up to light translucent wall panel. Outside of building- 1 • Clean efflorescence and restain these areas of emu. 2 • Clean soffit panels at cashier's window. 3. Caulk at S.S. counter/window sill. 4 • Clean emu block and touch up stain as required (especially at northeast comer near the ground). 5 • Latch at the northwest gate next to the building needs adjusting. 6 • Check all gates to make sure they swing without hitting asphalt and all gate keepers work. 7 • Clean up job shack site and the grounds in general. 8 . Touch up canopy rust at the purlins. 9. Provide tension rods and turn buckles at the canopy. 1 0 • Adjust and wire lights at the canopy. 11 • Check that all exterior lights work including the light pole and the sign lights. 12 .Exterior painting to be accomplished in 60 calendar days or as coordinated if weather does not permit. 13 • One canopy roof panel at the blue still needs some work. 14 • Verify irrigation system is installed properly and is completed.
