PROFESSIONAL SERVICES SUPPLEMENTAL AUTHORIZATION AlA DOCUMENT C604 (Instructions on reverse side) In accordance with the written Agreement dated as of June 4, 1991 between the parties designated below for THE PROJECT: Idaho Community Development Block Grant funded installation of features to provide barrier-free access, including the installation of a four-stop elevator, construction of a second stair, and remodeling of restrooms, in the former federal post office building located at 206 East 3rd in Moscow, Idaho. The Owner hereby authorizes the Architect Ga Proceed with Additional Services 0 Proceed with revised scope of services 0 Incur Reimbursable Expenses 0 to: as follows: Provide Architectural/Engineering services for remodeling the South Seventy of the SECOND FLOOR of the OLD POST OFFICE, including As-Built verification of existing, Programming, Schematic Design, Design Development, Construction Documents, Bidding or Negotiation and Construction Phase. Services are to be provided concurrently with the Idaho Community Development Block Grant project.
