Phoenix Environmental, Inc. Addendum to Agreement for Professional Services: Asbestos and Lead-based Paint This is an addendwn to the agreement entered into on October 2, 2000 between Phoenix Environmental, Inc. (Phoenix Environmental) and the City of Moscow (Client). Under the original contract, Phoenix Environmental agreed to perform a survey to identify and sample for asbestos containing building materials and lead-based paint in the building located at 410 East Third Street, Moscow, Idaho, hereinafter referred to as the "1912 Building". Additions to Original Scope of Work At the Client's request, the scope of the project has been expanded to include the following services: • Assist the Department of Corrections (DOC) Project Supervisor in developing a work plan for compliance with OSHA regulations (29 CFR 1926.62). Phoenix Environmental, Inc. is not responsible for final approval of the work plan, enforcing the work plan, or supervising or training workers. • Perform personal air monitoring for lead exposure for DOC workers. The amount of monitoring required for the project will be based on the different types of tasks, the types of materials being demolished, and the lead concentrations in surface coatings. • Sampling demolition debris either in place or after removal for hazardous waste characterization (TCLP). The nwnber of samples collected for analysis will depend on how the demolition tasks are grouped and the presence of lead-based or lead-containing coatings, as determined during the inspection preceding demolition. • Performing additional sampling and analysis (beyond the scope of the original inspections) of materials suspected of containing asbestos or of surface coatings suspected of containing lead discovered during the course of demolition. These services are expected to be carried out from October 2000 through February 2001, as pre renovation demolition is performed. Most of these tasks are expected to be performed early in the demolition phase of the project. Because of the extended time-frame, additional tasks might be encountered later in the project. Costs for services added to the original scope of work Costs for these added services are in addition to the estimated cost of the original scope of work. Activities listed above will be billed at tbe following rates. These rates are consistent with those charged under the original agreement. Hourly rate for consulting, monitoring and sampling activities, data evaluation, reporting: $45/hr Sample analysis (all samples will be sent to a NLLAP/NVLAP accredited laboratory for analysis): • TCLP analysis for hazardous waste characterization: $55/sample • lead air filter and analysis for personal air monitoring: $8/sample Special equipment: • personal air monitoring $35/pwnp/day, $75/pwnp/week • air monitoring pwnp primary standard calibrator: $75/day, $!50/week Addendwn to Agreement for Services Page 1 zooo-71 ---PAGE BREAK--- Phoenix Environmental, Inc. Addendum to Agreement for Professional Services: Asbestos and Lead-based Paint Additional costs: sampling supplies and equipment, copies, expedited sample shipping, report production Estimated Cost Charges for original tasks and tasks added under the expanded scope of work will be included on the same invoice but will be identified separately as original scope charges or expanded scope charges. The estimated cost of the described scope of work is $3,800 to $1 0,235; the actual cost will not exceed $10,235. The basis for the estimated eost is ineluded in the attached "Estimated Level of Effort (LOE)" table. The LOE calculations are based on preliminary data results received so far and discussions with people involved with the project about expected tasks. The actual cost will depend upon time expended and the materials used by Phoenix Environmental in completing this scope of work. All other terms and conditions of the original agreement apply.
