June, 21,1999 CHANGE ORDER NO. 1 Proj ect: STREET STORM SEWER Mountainview Road West- Proj. 110-98 1. 4 INCH FOUNDATION CONNECTIONS Two foundation drains were discovered in the existing ditch and needed to be connected to the 30 inch storm pipe. 2. 6 INCH STORM DRAIN STUB SUBTOTAL . . . $ 280.00 The Bergman/Bradford lot split brought about the need to stub out one additiional 6 inch storm pipe which also required an "inserta-tee" into the 48 inch storm pipe. SUBTOTAL . . . . $ 210.00 3. CONCRETE ENCASEMENT It was necessary to encase the fire hydrant service line below the 30 inch storm sewer crossing. SUBTOTAL . . . . $ 238.00 4. CONCRETE SLAB In order to obtain a product guarantee on the 48 inch "wye" we had to construct a 5 inch thick concrete slab over the "wye" to help distribute possible future load on the welded fitting.
