DecemberS, 1999 CHANGE ORDER NO. 1 Berman Creekside Park - 1999 Project No. 110-99 1. Added excavation at the east end of the trail. The project's scope of work did not indude the excavation of an existing slope that was needed to make room for the new path's typical section. Add $1 ,500.00 2. Move existing driveway. An existing driveway to Brian Claypool's residence needed to be moved to allow him better access to the new park entrance drive. This work had not been anticipated and was not included in the original design. Mr. Claypool in exchange for this work did abandon an existing access easement on park property - to the benefit of the City. Add $1,000.00 3. Raise parking Jot grade. The northeast comer of the parking lot needed a field adjustment to it's plan design grade - to keep an adjoining ditch from draining into the parking lot. An embankment of 2" ballast was constructed. Add $195.00 4. Raise path grade to meet curb. The path grade and the curb grade did not meet properly where the two came together at the turnaround - a field adjustment to the elevation of the path was accomplished by adding rock base and asphalt along a short distance of the path. Add $530.10 5. Install conduit sleeves. 5 conduit sleeves were installed across the path at various locations for future irrigation line use. This work was not included in the original scope of work. 6. Soft spot repair. Some of the subgrade for the driveway and the path was too soft to compact and had to be replaced with a foot of crushed rock over geotextile. This work had not been anticipated and was not described in the original scope of the contract.
