February 27, 1996 ' CHANGE ORDER NO.4 Project: Styner Avenue.- L.I.D. No. 113 1. Lower Water Line a. Lowering the parking lot at 123 Styner has created a very shallow depth on the water service line. Since the line is on the building side of the meter, the City does not make these adjustments. No provision was made in the plans or specifications to cover this work. The contractor has agreed to do the work for $610 to lower approximately 36 I. f. of 1" water line. Lower Water Line . . . . . . . . . . $610 2. Construct Irrigation Line a. An irrigation line was discovered crossing the parking lot at 1410 So. Main Street (dental clinic} as the grade was being lowered. The contractor will need to make the connections on both ends, furnish and lower the 1" PVC pipe. The contractor has agreed to do the work for $500. 3. Adjust Fire Hydrant SUBTOTAL . . $500.00 a. The water line had to be lowered thru the Styner -Indian Hills Drive intersection because of a grade conflict with a sewer line. As a result, the fire hydrant assembly tap, lateral and hydrant also ended up deeper than expected and must be adjusted with a 12" riser kit. 4. Slotted Drain Pipes SUBTOTAL. . $353.63 a. The concrete driveways being poured at 413 and 415 Styner are slopP.d toward the building, with no way to keep surface water from running into the garage and/or foundation walls. We propose to have the contractor furnish and install slotted drains into the concrete slab which will intercept water before it reaches the building. 5. Repair Soft Spots SUBTOTAL. . . . . $ 1 , 683.62 a. Prior to placing base aggregate on the subgrade we must have the contractor dig out soft areas in the roadway prism and place a geotextile fabric under approximately one foot of stabilization rock. Excavation, rock and fabric costs quoted for this work equate to $4.94/ s.y. which is a fair price for 380 s.y. SUBTOTAL. $ 1,876.45 96-08 ---PAGE BREAK--- C.O. No. 4 can't 6. Additional Construction Staking a. When soliciting quotes for construction staking, we did not specifically request that staking of approaches be included unless they fell on an even 50 foot station. In the field we found it necessary to request that most of the approaches be slope staked. DEBCO's sub-contractor billed them $1,013.44 to do this work.
