September 8, 1997 CHANGE ORDER NO. ONE Project: MOUNTAINVIEW BRIDGE - 1997 Project No: 102-97 1. Concrete encase a storm culvert connection band joining an existing 36-inch culvert to a new 36-inch culvert. The encasement was not included in the original scope of work but was needed to ensure the stability of connection. Add $135.77 2. Install 60 feet of 12-inch D.l. watermain under the new culverts for a future new 12-inch watermain along Mountainview. This work was not included in the original scope of work but was determined to be necessary when the existing watermain was found to be a shallow bury 8-inch lead joint pipe. Add $6,170.47 3. Furnish and install additional rip-rap to better accommodate the site conditions. The rip rap specified was of insufficient quantity. Add $316.1 4 4. Additional excavation and backfill to construction a stable shoulder along the southwest quadrant of the bridge. The existing subgrade at this location was too soft to allow adequate compaction for the specified shoulder widening.
