October 13, 2008 CHANGE ORDER NO. ONE South Lift Station-2008 1 . Increase the height of the standby generator building by 16 inches. The additional height was required to accommodate the generator manufacturer's recommended clearance to the building's ceiling; the design plan dimension would not have allowed the proper clearance. Added Cost $1 ,081.00 2. Replace the existing posts in a 118 foot long section of existing chain link fence. Plans and specifications required only the fabric along this 118 foot long section of existing chain link fence to be replaced; the posts were to be salvaged- in place. Due to the excavation limits required to accomplish nearby underground plumbing the posts had to be removed and replaced with new. Added Cost $1,6 91 .9 5 3. Furnish and install crushed aggregate backfill in the area of the new and the existing 1 0" gravity sewer connection; plus three hours of excavating rock to accomplish this connection. a Native baclifill had originally been specified in this area but was changed to crushed basalt to avoid future settlement issues . b. Large boulders were encountered; rock excavation had not been anticipated and was set forth in the contract documents as 'extra' work. Added Cost $2,708 .2 5 4. Furnish and install field lo k gaskets in the push together joints of the new 1 0 inch diameter force main. These accessory gaskets -were not originally specified but were added as a precautionary means of deterring future pipe failure. Furnish and install four, galvanized gate valve nut operator extensions in lieu of painted ones. The four, new gate valve operating nut extensions in the project were originally specified to be only painted. A change was made to have them galvanized in order to ensure their longevity. Added Cost . $1,771 .00 6. Furnish and install one additional 22.5 degree bend fitting in the new 1 0 inch diameter force main. The additional fitting was required to accomplish the vertical and horizontal plumbing configuration necessary to plumb the new force main to its connection with the existing force main. Furnish and install one area drain between the Fountain Industrial Park entrance road and the new lift station fencing. This drain is simply a replacement and improved version of an existing drain that had been placed there previously by others; it serves to convey surface runoff drainage from both the new l((t station site and the Fountain Industrial Park entrance driveway. Furnish and install87 tons of %''(-)crushed basalt gravel surfacing over the entire lift station site. The project documents had specified that the material from the on-site excavations would be stockpiled on the south side of Fountain's runway near the north bank of the South Fork of the Palouse River and any excess not subsequently later utilized as project backfill was to remain there and simply graded to blend with the north river bank( a plan approved by Fountain's). Upon excavation the material was found to contain significant quantities of the concrete and asphalt chunks which prohibited it from being utilized as permanent river bank embankment. The Contractor was then directed to truck haul the stockpile of unused excess excavation to the City's dump site. Furnish and install conduit and conductors from motor control center to generator set. This mandatory item was inadvertently left out of the original design documents. Added Cost $333.50 11.
