ADDENDUM NO.2 TO BID PROPOSAL AND CONTRACT DOCUMENTS FOR THE PLAYFAIR PARK SIDEWALK IMPROVEMENTS - CITY OF MISSOULA CTEP PROJECT PROJECT NO: STPE 8199(86) MDT CONTROL NO: 6764 NOVEMBER 24, 2009 In accordance with the INSTRUCTIONS TO BIDDERS of the Contract Documents, the following revisions, additions, deletions and clarifications shall become part of the Contract documents, and bidders shall acknowledge receipt of this addendum on form "ACKNOWLEDGMENT OF RECEIPT OF ADDENDUM" contained in the contract documents. Bid will be considered non-responsive and rejected if receipt of each addendum is not acknowledged. The following list consists of one addendum and three informational items. ITEM NO.1 (addendum) The bid opening has been re-scheduled to December 2009 at 1:30 PM in the Mayor's Conference Room. Sealed bids must be received by 1:30 PM at the office of the City Clerk, 435 Ryman Street, Missouia, Montana. ITEM NO.2 (addendum) See attached new Unit Price Bid Sheet. Some quantities have changed. The new quantities are listed below. A11 - Minus Crushed Base Course, 160 CY A12 - Type Curb and Walk (4"),592 LF A14 - 4" Thickness Concrete Sidewalk, 3336 SF A15 - 6" Thickness Concrete Sidewalk, 605 SF A16 -ADA Ramp, 4 EA ITEM NO.3 (addendum) Note: 1) Detail E Sheet 02 stationing has changed to 1+70 to 8+17 2) Detail F Sheet 02 stationing has changed to include 8+17 to 8+71 ITEM NO.4 (addendum) The current Montana Prevailing Wage Rates for Heavy Construction are attached in this addendum. Piease be sure to use these wage rates. ITEM NO.5 (information) The approximate square footages for the ADA Ramps have been added to the item description in the TLI Special Provisions. A copy of this change made to the Special Provision is attached. A detail for the Type ramp similar to the proposed ramps is also attached. ItemA11: 3/4" Minus Crushed Base (For Sidewalk and Access Aprons) 02235 - Shall be placed in accordance with 02235 at the depths indicated on the details provided in the Construction Plans. Compensation for this item includes all labor, tools, equipment, and other incidentals necessary to complete the above described work. Payment shall be in accordance with 02235, PART 4, 4.1, A, CRUSHED BASE COURSE - CUBIC YARD (CY) BASIS. Item A12: 4" Combined Concrete Sidewalk and Curb (Type - Shall be supplied and placed in accordance to construction plans. This item maybe measure and paid for at the contract unit price bid per lineal foot, constituting full compensation for all equipment, toois and labor, including the performance of all work to provide incidentals necessary to complete this item. Payment shall be - liNEAL FOOT (LF) BASIS Item A14: Concrete Sidewalk 02529 - Shall be supplied and placed to a 4" finished thickness in accordance with 02529. Payment shall be in accordance with 02529, Part 4,4.4, CONCRETE SIDEWALK- SQUARE FOOT (SF) BASIS including preparation, placement, and finishing. Item A15: Thickened Concrete Sidewalk 02529 - Shall be supplied and placed to a 6" finished thickness in accordance with 02529. Payment shall be in accordance with 02529, Part 4, 4.4, CONCRETE SIDEWALK- SQUARE FOOT (SF) BASIS including preparation, placement, and finishing. Item A16: ADA Ramp - Shall be supplied and placed in accordance with the details provided in the Construction Plans. Compensation for this item includes the labor, materials, and incidentals for properly constructing the ADA ramps. A similar Type ramp detail, including retaining curb, is attached behind these TLi specials.
