A representative from the Town will contact you to review your request. NAME: Last First Address of Proposed Planting: Email Please sketch the house/building, sidewalk, proposed location for the tree(s), and any other significant details of which the Contract Arborist should be aware on the reverse side of this form or on an attached sheet of paper. Pounding a stake (preferably flagged) in the proposed tree location is suggested.) Please be advised that this sketch is all the installer will have if the proposed location qualifies for the program. Because of the number of orders, the landscape contractor will not call before planting the tree. If possible, please include measurements in feet and inches to ensure proper placement of the tree(s).
