Business License Application Procedure & Contact Checklist PLANNING & ZONING: Planning will review your application for zoning and approved uses. After receiving Planning's approval your application will be forwarded to the remaining agencies, and you can begin calling to schedule those inspections. MILLCREEK BUILDING SAFETY: When your place of business is ready for a Building Safety Inspection, please call (801) 214-2724 to request a “Business License Inspection” to be scheduled. Note: Inspections are scheduled as an AM or PM rather than a specific time. You will need to be available at the property during the time given. UNIFIED FIRE AUTHORITY: Please call Shirl White at (801) 824-3717 and/or email: [EMAIL REDACTED] to request a Fire Safety Inspection. -----OTHER AGENCIES THAT MAY NEED TO INSPECT AND APPROVE YOUR BUSINESS----- SALT LAKE COUNTY HEALTH DEPARTMENT: If your business requires approval from the Health Department, we will send a copy of your business license application to them. You are required to contact the Health Department to find out what requirements/permits will be needed for your business. Your license cannot be issued until we receive confirmation from the Health Department of approval. Health Department Contact Number: (385) 468-3860 UTAH DEPARTMENT OF AGRICULTURE AND FOOD: If your business requires approval from the Utah Department of Agriculture and Food we will send a copy of your business license application to them. You are required to contact this Department to find out what requirements they may have for your type of business. Your license cannot be issued until we receive confirmation from the Department of Agriculture and Food of their approval. BUREAU OF ALCOHOL, TOBACCO, FIREARMS AND EXPLOSIVES: If your business requires approval from the Bureau of Alcohol, Tobacco, Firearms and Explosives we will send a copy of your business license application to them. You are required to contact the Bureau to find out what requirements they may have for your type of business. Your license cannot be issued until we receive confirmation from the Bureau of Alcohol, Tobacco, Firearms and Explosives of their approval. CODE ENFORCEMENT: Once all other agency approvals are received on your application, Business Licensing will forward your application to Code Enforcement for the final inspection & approval before issuing your license. SUBMIT COMPLETED APPLICATION (Submitted application will be reviewed by Business Licensing before being accepted.) 2. APPLICATION FEES (Before any inspection/approval can occur, payment for application must be received. Link for payment will be emailed to applicant.) 3. APPLICATION FORWARDED TO PLANNING & ZONING FOR APPROVAL (Business Licensing will forward the application to Planning. No other inspections can be scheduled until Planning's approval has been submitted to Business Licensing.) 4. APPLICATION FORWARDED TO OTHER AGENCIES FOR INSPECTION APPROVAL (You must call these agencies to schedule your inspections after approval from Planning has been received.) Each business type outlined below will require the following inspections; a. Retail/Office: Building Departments, Fire Department b. Restaurant/Salon/Spa: Building Department, Fire Department, SLCO Health Department, DABC (only if liquor is sold) c. Rental Property: TBD (depends on type of rental) d. Home Business: TBD (depends on type of business) e. Seasonal/Solicitor: TBD (depends on type of business) 5. APPLICATION FORWARDED TO CODE FOR FINAL APPROVAL (Once the applicable inspection approvals have been received, the application will be forwarded by Business Licensing to Code for the final approval.) 6.
