All Millcreek residents have access to healthcare 2. All Millcreek residents have resources for mental wellness and, if necessary, access to mental health services 3. All Millcreek residents have access and opportunity for physical fitness. 4. All Millcreek residents have access to high-quality nutrition. 5. Special area of emphasis: Suicide prevention ---PAGE BREAK--- BASELINE DATA INDICATOR 1: All Millcreek residents have access to healthcare Proxy data: % of individuals that have no health insurance Desired Result: 1% annual total population decrease = 621 fewer uninsured individuals per year Proxy data: % of individuals that have no health insurance coverage ---PAGE BREAK--- BASELINE DATA INDICATOR 2: All Millcreek residents have resources for mental wellness and, if necessary, access to mental health services Data: % of individuals that report seven or more days of poor mental health in the past 30 days Desired Result:1% annual decrease = 647 will report less than 7 days of poor mental health in the past 30 days. ---PAGE BREAK--- BASELINE DATA INDICATOR 3: All Millcreek residents have access and opportunity for physical fitness. We are hoping to continue to acquire data in all indicator areas to inform both the baseline and formative data. ◼Additional committee members with expertise, experience or passion for each of the specific indicator areas- commitment 2-4 hours per month ◼Healthcare access, Mental Health, Fitness, Nutrition ◼Especially would appreciate committee involvement from Salt Lake County Health Dept., participation of pharmacy managers, addition healthcare, mental health, fitness and nutrition practitioners.
