Millcreek Volunteer Contract VOLUNTEER CONTRACT I agree to perform the volunteer services as specified below, to the best of my ability and in a professional manner. VOLUNTEER SERVICES: I understand my duties as a volunteer include, but are not limited to, duties performed as a member of the (Name of Volunteer Organization) CONFIDENTIALITY: I agree to maintain the same strict confidentiality regarding my duties that is expected of the paid staff. RELEASE: While performing volunteer work assignments and duties, the undersigned volunteer (unsalaried worker) shall be deemed an employee of Millcreek only for the purpose of the following liabilities and insurance coverage: A. Benefits under Worker’s Compensation for any injury sustained by him/her while engaged in performance of any service; B. Properly licensed operation of Millcreek vehicles or equipment; C. Liability protection normally afforded salaried employees. I acknowledge that Millcreek accepts no other liability than that stated above. With this knowledge, the undersigned volunteer hereby releases Millcreek, its agents and employees from any liability or obligation arising from, or in connection with, the undersigned’s volunteer services as stated above. I have read the information entitled Sexual Harassment and Discrimination Prevention for Volunteers and understand it or have had an opportunity to ask questions. I have read the Statutory Ethical Review Form and I understand it or have had an opportunity to ask questions. I have read and understand the above conditions.
