Millcreek Planning and Community Development 3330 South 1300 East Millcreek, Utah 84106 Phone: (801) 214-2750 Inspections: (385) 468-6690 Version: 1 December 2018 - Page 1 of 2 Millcreek Sidewalk Curb and Gutter (C&G) Cost-Sharing Program Purpose The primary purpose of the Millcreek Sidewalk/C&G Cost-Sharing Program is to assist Millcreek residents in installing new Sidewalk/C&G or repairing or replacing existing Sidewalk/C&G adjacent to their homes by reimbursing eligible residents up to 50 percent of the total cost of the project. Project Eligibility, Requirements, and Reimbursement Sidewalk/C&G installation, repair, or replacement projects must be in a Millcreek residential neighborhood and within a Millcreek-owned right-of-way. To be eligible for reimbursement, the applicant must be a current Millcreek resident, reside in the home located on the property, and the property must not be subject to a delay agreement or a required improvement by the Millcreek Code of Ordinances or the Planning Commission. Standards All Sidewalk/C&G installation, repair, or replacement must comply with current Millcreek engineering standards, which are attached to this application. Millcreek engineering standards include but are not limited to a requirement that the contractor delivers to Millcreek a payment bond and performance bond in an amount equal to 100% of the price specified in the bid. See Utah Code Ann. § 63G-6a-1103. Outline of Steps 1. Complete and submit a Millcreek Sidewalk/C&G Cost-Sharing Program Application. Provide an improvement plan identifying the proposed improvements that includes a drawing with measurements. The applicant must complete and submit a Millcreek Sidewalk/C&G Cost- Sharing Program Application before proceeding to next steps. If applicant obtain bids or permits prior to submitting the application, Millcreek will not participate in cost-sharing or issue a reimbursement upon completion of the project. 2. Millcreek Staff will review eligibility and improvement plan. If the project is eligible and funds are available, Millcreek will review the improvement plan for compliance with applicable engineering standards and will work with the applicant as necessary to revise the improvement plans to meet engineering standards. If the plans are approved, Millcreek will notify the applicant to obtain bids for pricing. 3. Obtain bids. The applicant will obtain a bid or bids (preferably three bids) and provide Millcreek with the bids. Bidders must be contractors licensed by the State of Utah. Millcreek will review the bids. 4. Approval. If Millcreek finds a bid acceptable then Millcreek will notify the applicant of approval. 5. Permits and Bonds. Prior to beginning work the applicant or its contractor is required to obtain a Right-of-Way Improvement Permit from Millcreek Permits. The contractor is required to provide Millcreek with a performance and payment bond. 6. Completion. Once the project is complete, submit all invoices to Millcreek for reimbursement. Millcreek will schedule an inspection of the project.
