Affected pages are not being mailed, but are available on the City’s web site. Effective immediately, the following changes should be made to various contract documents for public works projects in the City of Middleton: Table of Contents (Page TC-1) The note at the bottom of the page has been updated to indicate that the standard documents have been revised to be current as of January 7, 2008. Agreement (Page A-5) The referenced general conditions and specifications are now noted to be in the City of Middleton Standard Specifications and Construction Contract Documents, dated June 2004, and most recently revised January 7, 2008. General Requirements (Page 100-14) Paragraph 126.I has been changed to require timely replacement of concrete within the limits of a driveway. Storm Sewer (Page 300-4) Paragraphs 301.G and 301.H have been edited to redefine the type of structure labeled as a catch basin. Water Main (Pages 400-2 and 400-3) New paragraph 401.A.2 has been inserted to describe the intended materials for water main joints. Sanitary Sewer (Pages 500-4 and 500-5) Paragraph 501.F has been reformatted and modified to approve an additional lining product. Sanitary Sewer (Page 500-6) Paragraph 501.J has been modified to clarify that exceptions may be made to the requirement for providing an internal chimney seal at every sewer manhole. Flexible couplings may still be used for sewer lateral repairs if rigid couplings are not available. Street Construction (Page 600-8) Paragraph 609.E.2 has been modified to clarify that concrete cylinder sets need not be made from every truck, but are intended to be made only from a representative number of trucks. Street Construction (Page 600-9) Paragraph 609.H.4 has been modified to require sawed contraction joints in concrete sidewalks intended as multi-use paths. This is intended to make the paths better suited for rollerblading. Street Construction (Page 600-11) Paragraphs 610.B.5 through 610.B.7 have been combined into a new paragraph 610.B.5, which now includes also clarifying language regarding removal of existing adjusting materials prior to installing HDPE adjusting rings. Paragraph 610.C.3 has been modified to remove the requirement for PG 64-28 asphalt binder in the upper layer placed in the same year as lower layers. Standard Detail Drawing 100-15 Several clarifying notes have been added. Standard Detail Drawing 300-6 A new drawing of a catch basin structure has been created. Former detail drawings 300-3 through 300-6 have been renumbered to 300-2 through 300-5. Standard Detail Drawing 400-2 A note has been added to clarify that adjustment rings are to be configured to minimize the number required. Standard Detail Drawing 500-1 A note has been added to clarify that adjustment rings are to be configured to minimize the number required. Standard Detail Drawing 600-4 A note was added for the minimum width of commercial driveways in the event the drive is one way. A note was added to specify the range of radii to be used for Type B aprons.
