Effective immediately, the following changes should be made to various contract documents for public works projects in the City of Middleton: Overview of Contract Documents (Pages P-3 through P-5) A section has been added to indicate how the typical project manual is intended to be assembled from the standard specifications and construction contract documents. Wage Rates (Page W-1) The fourth paragraph has been changed to reflect that the construction cost threshold changes annually, and that the form included in the appendix may not be the most recent. The web address to download the current form has also been changed. Table of Contents (Page TC-1) The note at the bottom of the page has been updated to indicate that the standard documents have been revised to be current as of February 2, 2006. Advertisement to Bid (Page AB-1) For City projects, the Advertisement to Bid is published under the authority of the City Clerk. The City Clerk is currently Patricia A. Amble. Instructions to Bidders (Page I-7) Article 15 of the Instructions to Bidders has been modified to add paragraph 15.03, stating that any submissions to the City connected with the project are subject to disclosure in accordance with a public records request. Agreement (Page A-5) The referenced general conditions and specifications are now noted to be in the City of Middleton Standard Specifications and Construction Contract Documents, dated June 2004, and revised February 2, 2006. Agreement (Page A-8) City contracts are signed by the mayor, whose signature is attested by the City Clerk. The City Clerk is currently Patricia A. Amble. Construction Performance Bond (Page CP-1) For City projects, the performance bond is approved by the Finance Director. Tim Studer is no longer also the City Clerk. The title of the owner approval signature line has replaced the title “Finance Director / City Clerk” with the title “Finance Director”. Construction Payment Bond (Page CP-5) For City projects, the payment bond is approved by the Finance Director. Tim Studer is no longer also the City Clerk. General Requirements (Page 100-1) The Table of Contents for this section has been modified to indicate that Section 129 starts on page 15. Water Main (Page 400-4) Paragraph J has been added to Section 402 to prohibit placing water service connections within 2 feet of a water main joint. Water Main (Page 400-5) The requirement for electrical conductivity testing (paragraph 403.F.2) has been replaced with a requirement for water main locating (continuity). Street Construction, Sitework and Restoration (Section 600) To remain consistent with changes made to the WisDOT Specifications, Section 607 of the City Specifications has been re-written to refer to dense graded base instead of crushed aggregate base course, to adopt the WisDOT gradation requirements, and to correct the reference to the pertinent WisDOT section number. In making the change to dense graded base, references were also changed in Standard Detail Drawings 600-5, 600-7, 600-9, 600-10, 600-12 (now 600-13), and 600-13 (now 600-14) and to paragraphs 609.G(2), 609.H(2), 613.B(2), and 613.C(9). To remain consistent with changes made to the WisDOT Specifications, Section 610 of the City Specifications has been re-written to correct the reference to the WisDOT section number, to modify standard asphalt binder materials, and to add a requirement for tack coat materials. New Standard Detail Drawing 600-11 has been added to show standard pavement joints. Subsequent standard detail drawings were renumbered. To remain consistent with changes made to the WisDOT Specifications, references to pertinent WisDOT section numbers in the City Specifications have been made in paragraphs 608.A, and 609.D(10)(a). Clarified the minimum size of planting pits in paragraph 614.H(4), and added a new requirement in paragraph 614.H(9) to provide adequate topsoil areas for trees in new or reconstruction projects. ---PAGE BREAK--- C:\Program Files\Neevia.Com\Document Converter\temp\Amend 2006-02-02.doc Page 3 of 4 1/15/2 Standard Detail Drawing 100-4 With the County’s review of erosion control plans, their stated preference is to use Type D inlet protection, instead of Type C, whenever possible. The detail drawing for Type C inlet protection has been replaced with the new standard Type D. Standard Detail Drawing 300-2 The standard sump depth of a catch basin is 2 feet. The previous detail drawing incorrectly showed it as 1.5 feet. Standard Detail Drawing 300-6 To facilitate sediment removal from manhole structures with sumps, they should be constructed with concentric cones instead of offset cones. The detail note has been changed to effect this requirement. Standard Detail Drawings 300-3, 300-4, 300-7, 300-8 There has been some confusion about which side of the plastic adjusting rings should be placed to face up. The detail drawings have been revised to label the top and bottom sides of the rings. Standard Detail Drawing 400-3 The revised drawing more clearly shows the intended minimum cover over the water main and water lateral. Minimum spacing of corporation stop taps from water main joints has been changed to 24”, consistent with changes to water main specifications above. Standard Detail Drawing 600-3 A minor wording edit was made to the third note, to clarify the intent. Standard Detail Drawing 600-4 Similar to the detail drawing for residential driveway aprons, the commercial driveway apron detail now includes an option for a flare and taper, instead of just a radius transition. Notes were also added, consistent with the residential driveway apron drawing. Standard Detail Drawings 600-5, 600-7, 600-9, 600-10, 600-13, and 600-14 As noted above, former references to crushed aggregate base course have been replaced with references to dense graded base. The affected detail drawings have been modified to reflect this change. Due to creation of a new detail drawing 600-11, subsequent detail drawings have been renumbered. Standard Detail Drawing 600-11 A new detail drawing has been added to detail the longitudinal joints to be used for asphalt pavement, consistent with DOT specifications. The detail drawing also shows end and edge joints to be used for asphalt overlay projects. Water Main Pressure Test Report (Appendix) The format of the first page of the water main pressure test report has been modified and the spaces for recording test information have been enlarged.
