Affected pages are not being mailed, but are available on the City’s web site. Effective immediately, the following changes should be made to various bidding and contract documents for public works projects in the City of Middleton: Wage Rates (Page W-1) References to Dept. of Workforce Development forms are included by number, and it is noted that the most current version of the various forms are available on the DWD web site. Table of Contents (Page TC-1) The note at the bottom of the page has been updated to indicate that the standard documents have been revised to be current as of January 9, 2009. Advertisement to Bid (Page AB-1) A line has been provided to indicate the MSIP project number. This should be edited if the project is being partially funded by the State MSIP program, and should be deleted if MSIP funding does not pertain to the project. Agreement (Page A-5) The referenced general conditions and specifications are now noted to be in the City of Middleton Standard Specifications and Construction Contract Documents, dated June 2004, and most recently revised January 9, 2009. Payment Bond (Page CP-7) The City’s payment bond form is based on standard EJCDC form C-615(A). As an interim measure, paragraph 6 has been deleted, and is now listed as reserved. The former provision in paragraph 6 required the surety to provide a written response to claimants within 45 days of receipt of the claim and to pay any undisputed amounts. Street Construction (Page 600-2) Paragraphs 602.B and 602.C have been modified to clarify the requirement to remove stumps and roots within 12” of finish grade, and to restore areas of tree removal to finish grade. Street Construction (Page 600-10) Paragraph 609.I.2 has been modified to add MD Meta Panel as an approved material for curb ramp detectable warning fields. Street Construction (Page 600-12) Paragraph 611.C has been modified to eliminate epoxy pavement markings, and to specify instead paint and glass beads. Street Construction (Page 600-13) Paragraph 612.B.3 has been modified to specify non-phosphorus fertilizers for grass restoration. Appendix Table of Contents Department of Workforce Development (DWD) forms have been eliminated from the appendices. These forms change on an annual basis, so reference to them on page W-1 will refer preparers of project manuals to the most current forms.
