Healthy Communities Challenge Day 10am – 2pm Saturday, June 2 Allen Creek Elementary School, 6505 60th Dr. NE Exhibitor Application Welcome to the 10th annual Healthy Communities Challenge Day! Please complete the information below for consideration in this year’s event. For marketing purposes, applications are due by 5pm Monday, April 30. The nature of this event is to get participants active and engaged with the vendors. (Not just picking up pamphlets or coloring sheets.) $20 donations are being accepted to offset event costs. I have read and understand the Healthy Communities Challenge Day guidelines as stated below. For and in consideration of the opportunity offered to participate in Healthy Communities Challenge Day I, as evidenced by my signature below, do hereby hold harmless, release and waive all claims I may have against the Healthy Communities Challenge Day Organizers: City of Marysville, YMCA of Snohomish County, Marysville School District, and Marysville Together Coalition, it’s officials, employees, agents, or contracted instructors, and any other person involved for any and all injuries, losses or damages suffered by me as a result of my participation. I accept full responsibility for the cost of treatment for any injury, losses or damages suffered. I grant the Healthy Communities Challenge Day Organizers: City of Marysville, YMCA of Snohomish County, Marysville School District, and Marysville Together Coalition, it’s representatives and employees the right to take photographs of me and my property in connection with the above-registered program. I authorize the Healthy Communities Challenge Day Organizers: City of Marysville, YMCA of Snohomish County, Marysville School District, and Marysville Together Coalition, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I have read and understand the above. Signature Date Event Details: Exhibitors donations are appreciated. There is not an entry fee for participants. Exhibitor’s donations go toward costs associated with the event such as table/chair rentals, facility fees, entertainment, door prices, marketing and other related items. Guidelines: If you want to offer food or food products, please speak with the organizer. All exhibitors must stay for the entire duration of the event 10am-2pm. Set up will be between 8am and 9:30am. Break down will begin at 2pm and we request you be finished by 3pm. This is a free event for participants. Please no sales at the event. Attendance at the event cannot be predicted. Press releases and other forms of advertising will be provided by the Healthy Communities Challenge Day planning committee. Exhibitors are also encouraged to advertise the event. This is an “activity” fair. All booths must have FREE on-site services or a FREE activity that engages the participant. COMPLIMENTARY take-away items are also permitted (no candy). In an effort to offer variety in exhibitors applications will be screened and you will be notified if your organization has been accepted. This is a family-friendly event; take-away items, on-site services and activities should be family-friendly.
