To: Rental Customer From: Marysville Parks, Culture and Recreation Department Subject: Insurance and Banquet Permit Information We look forward to working with you at your upcoming event at the Marysville Opera House. This memo will provide information to you about the additional paperwork needed for the alcohol (beer, wine and/or champagne only) component of your rental. Below is the information for two websites where you may go to purchase the permit and insurance. To purchase a banquet permit go to the state website: http://lcb.wa.gov/licensing/banquet-permits or call 1-[PHONE REDACTED]. The additional alcohol insurance needed may be purchased at www.ebi-ins.com/tulip. Once there follow the various TULIP steps using venue ID code 0465-560. If you have any questions, you may also contact their office at 1-[PHONE REDACTED]. Please remember that only persons 21 and older may consume alcoholic beverages. Proof of insurance for the alcohol liability as well as a banquet permit need to be delivered or mailed to our office at least 15 business days prior to your event.
