- Bid No: 2012-010 Bid Date: March 22, 2012 Date Issued: March 2, 2012 ADDENDUM 1 CITY OF LEWISTON ARMORY ELEVATOR ENCLOSURE BUILDING PROJECT DEPARTMENT OF PUBLIC WORKS This addendum is to be added to and become part of the Contract and Contract Documents and modifies the original document. Acknowledge receipt of this addendum in the space provided on the Bid Form. Failure to do so may subject the bidder to disqualification. It is the responsibility of General bidders to inform sub-bidders of any addendum provisions affecting their work. Notice to Contractors, Page N-1 Proposals will be received by the City of Lewiston, Maine at the office of the Director of Budget/Purchasing until 2:00 p.m., on March 22, 2012 April 12, 2012. Each proposal shall include a sealed fee statement under separate cover for the work to be performed. The fee statement will not be opened until the Design/Build Contractors have been rated. DIVISION 10 – 10.9 SPECIFIC PROJECT COORDINATION, Page 10-4 ADD: 10. Once the Armory Elevator Contract has been awarded by the City, the Design/ Build Contractors interested in submitting a proposal shall coordinate with the selected elevator supplier to determine the final details required to complete the Armory Elevator Enclosure Project Request for Proposal. The Design/ Build Contractors shall be notified by the City of the awarded amount and the Contractor shall include the cost of the elevator and installation with their proposal. The elevator system and installation will be the responsibility of the proposing Design/ Build Contractor, which will coordinate the storage, installation, testing and licensing of the new elevator. 11. The Design/ Build Contractor shall provide a concrete slab at the entrance to the elevator enclosure. The City of Lewiston shall provide access to the slab as part of its proposed Parking Facility Improvement Project. 12. The City shall retain 5% of the requested payments until the project has been 100% completed. 13. The electrical service for the elevator shall provide power for a maximum 30 horse power motor. 14. The City of Lewiston, at its expense, shall maintain Builders Rick Insurance for the Armory Elevator Enclosure Building Project. 15. The seismic design required must meet the State of Maine’s Building Code requirements. The City of Lewiston is an EOE.
