State/Federal Requirements: Davis Bacon, DBE, Buy America – the DBE Proposed Utilization form must be included with Bid (about page 47 of specs). DBE Attachment C must be included with every pay requisition – one for each PIN number, with the work performed and money paid to a DBE split accordingly, even if a DBE was not utilized during that time period. Wage rates have increased – see spec documents (about page 51 of specs) – this project is using General Decision #ME12049 1/6/12 – Highway. Only the bid forms need to be submitted, not the contract forms. Time Limit: July 31, 2012 (In advance of MaineDOT project on Route 196) – paving on the Lewiston/Lisbon project should begin on the Lisbon side and progress towards Lewiston. Project Limits: Westminster Street in Lewiston to 0.1 miles north of River Road in Lisbon approximately 6.33 miles. Scope of Work: Milling 1.25” of pavement, spot shim roadway, overlay with 1.25” of Hot Mix Asphalt. – this is not a full shim project. There are two PIN numbers for this project – though they are being bid as one project, billing will need to delineate between the two numbers. The split is at the Lewiston/Lisbon town line. ---PAGE BREAK--- There are a lot of manholes and catchbasins in the roadways. No catch basins need to be adjusted. There are sewer, storm, and Fairpoint manholes. The contractor will mill around the existing structures, those that are low and need to be adjusted will be adjusted by the respective utility, this can be determined during the pre-utility walk through. Lift rings will be used as much as possible to speed up the adjustment process. Item 202.2 Remove Pavement Surface shall be used on the streets including side streets, Item 202.203 Pavement Butt Joins will only be used in driveways that require overlaying, the intent is to match the existing driveways to the greatest extent practicable. Safety: The contractor shall be responsible to comply with all local, state, and federal regulations. – this area sees traffic of 13,000-14,000 vehicles a day and includes several intersections. Permits: The contractor shall be responsible for all permits including but not limited to: DIG SAFE, Excavation, Street Opening, Occupancy, Fill Sites – City issued permit fees will be waived. The contractor must go through the normal process for permits with the City, but the City fees will be waived. Traffic Control: Contractor is responsible for traffic control; special requirements include, but are not limited to: Two VMBs required for the project. (message boards). There are time restrictions for lane closures as noted below: PIN 19112.00 – Route 196 – Lewiston: One lane of traffic shall be maintained in each direction from 7am to 6pm, through the use of lane shifts. If alternating one-way traffic is necessary is shall be from 6pm to 7am for closure in excess of 1500 feet, (flagger to flagger). Alternating one- way traffic in closures shorter than 1500 feet may be done from 9am to 3pm. PIN 19124.00 – Route 196 – Lisbon: One lane of traffic shall be maintained in each direction from 7am to 6pm, with the following exception. Alternating one-way traffic, in lane closures no greater than 1000 feet in length, (flagger to flagger) may be done anytime except the afternoon peak hours of 3pm to 6pm. There cannot be any lane closures in Lisbon during the Moxie Festival weekend (7/13-7/15/12). Be aware of your signage – it is a very windy area and often prone to high speed traffic which may blow down signs. Please protect accordingly. Utilities: Aerial: No Adjustments required. Underground: Contractor is to mill around the existing manholes, only those manholes which will not be able to work due to grade (i.e. are already low) will be adjusted. Milling within the streets shall be paid for as item 202.2 Removing Pavement Surface, Item 202.203 Pavement Butt Joints shall only be for driveways. Lewiston Department of Public Works (Sewer) – Will adjust necessary sewer manholes to grade after the structure is milled around. Lewiston Department of Public Works (Water) – Will lower gate boxes prior to milling and will raise gate boxes during paving operations. Contractor shall provide offsets to gate boxes prior to paving. ---PAGE BREAK--- Lisbon Department of Public Works – Will adjust necessary sewer manholes to grade after the structure is milled around. Fairpoint – Will adjust necessary manholes to grade after the structure is milled around. Unitil- Will lower gate boxes prior to milling and will raise gate boxes during paving operations. Contractor shall provide offsets to gate boxes prior to paving. Contractor Questions: 1. What Type of camera is required for the video detection? - Traffic Cameras shall be Traficon Trafficam X-Stream. 2. Correction to page N-2 of the specs – the bid security should read five percent 3. Is Lewiston the contact for the work within Lisbon? -Lewiston is coordinating all the work on the project, all correspondence regarding the contract will go through Lewiston.
