The Finance Committee reserves the right to accept or reject any and all proposals. Please use a clearly marked envelope with the bid name and number when submitting your bid. Beauparlant Director of Budget/Purchasing NJB/syt ---PAGE BREAK--- CITY OF LEWISTON, MAINE DEPARTMENT OF PUBLIC WORKS WORK ZONE TRAFFIC CONTROL (FLAGGERS) BID 2012-019 BID DATE: April 24, 2012 SPECIFICATIONS The City of Lewiston, Department of Public Works is seeking bids for Work Zone Traffic Control for various construction and/or maintenance projects for a one year period beginning on the date of award of this bid. The following specifications shall be the minimum requirements. The factors to be considered in awarding the bid shall be services provided, cost and references. The City of Lewiston reserves the right to reject any and all bids, when deemed to be in the best interest of the City, and to waive any minor discrepancy or technicality in the bid specifications. Bids shall be submitted to the Director of Budget/Purchasing, City Building, Lewiston, Maine 04240 until 2:00 p.m. on Tuesday, April 24, 2012 and then at said office opened and read aloud. Questions on the bid shall be directed to David Jones, Director at the Department of Public Works, 103 Adams Avenue, Lewiston, Maine 04240, Telephone (207) 513-3003 Ext. 3313. The length of the contract shall be for a one year period, beginning at the time of award of the bid. The contracted firm shall: • Obtain, at time of contract, and continue to maintain, a minimum of five hundred thousand ($500,000) dollars in liability insurance throughout the entire duration of the contract; • Have Workers’ Compensation coverage with Statutory Limits & Employers Liability for all employees with limits of $400,000 per incident; • Meet the requirements of OSHA standard 1926.201 and Maine State Law Title 23 MSRA 707; • Provide properly trained and qualified employees who will place an emphasis on clear messages to motorists, allow safe distance for drivers to react and coordinate work with other flaggers in a safe and timely manner; • Provide and use OSHA approved reflective equipment, such as, hardhats, paddles, vests, lights (if applicable), etc. in good operating condition; • Perform said service with a good approach sight distance, highly visible to traffice ---PAGE BREAK--- and never standing in moving traffic lane; • Provide 2-way radios when requested or when deemed necessary by the Department of Public Works; • Provide employees who conduct themselves in a neat and professional manner, placing an emphasis on being courteous under any and all situations which may occur; • Be in compliance at all times with and continue to maintain it and all State and Federal Regulations which pertain to Work Zone Traffic Control as described in the “Manual on Uniform Traffic Control Devices”; • Provide staffing for the duration of the work shift and the duration of the project, as determined by the Department of Public Works; • Instruct employees that they shall coordinate their services in conjunction with the supervisor or his/her designee of the various construction and maintenance projects; • Provide the City with a list of five references for related work that includes names, addresses and telephone numbers of the firms, contact person and length of the services provided; • Remove any flagger(s) who are not performing to the standards set forth in this bid specification or as requested by the City, and replace them with qualified replacements; • Provide qualified flagger(s) within a sixteen (16) hour verbal notice from the City; • Preference will be given to firms providing flaggers certified under the Maine Department of Transportation (MDOT) Flagger Certification Program. Firms planning to provide flaggers certified under this program should make this known as part of their bid package and be prepared to provide a list of certified flaggers upon award of the contract; • Flaggers will report to the assigned job site on time and with appropriate clothing for the weather conditions and equipment to perform their job. The City of Lewiston, Department of Public Works reserves the right to determine which construction and/or maintenance projects this service will be utilized on and that said Department reserves the right to utilize its own employees at the Department’s sole discretion.
