January 29, 2014 East Ave Water Main Replacement Bid No. 2014-003 Bid Date: 2/18/14 ADDENDUM NO 1 Please make the following changes to the Specifications: ADD sewer profile and the following: The attached sewer profile is approximate based on GIS elevations, field manhole observations and a 1962 plan/profile. Record drawings indicate a change in grade 40’ from MH 23+16. The intent is to replace the sewer in its existing location, not to remove rock below the pipe to straighten the grade. The existing asbestos cement sewer shall be removed, transported and disposed of according to Maine DEP asbestos regulations, Chapter 425, and OSHA regulations. A health and safety plan addressing the handling and disposal of asbestos pipe shall be submitted prior to removal of any pipe. Asbestos pipe shall NOT be disposed of at the Lewiston Landfill, Lewiston quarry or any other site not licensed to receive asbestos. All costs associated the health and safety plan and removal, transportation and disposal of asbestos pipe shall be incidental to installation of the new sewer. Overall scope 2000 LF 12”, Fairlawn to Alec 1100 LF Alec to Montello 550 LF Harkins and misc. City to supply all water materials 3. Sequence of Construction ---PAGE BREAK--- 2 No temporary main. Services and sidewalk are new. Start at Fairlawn, locate existing 12”, cut and cap 6” fed from other end. First section is tight in the center of road between gas and old water but no ledge. Leavitt St water main to be replaced summer of 2014 as a separate contract. After Leavitt, cross to east side of road, begin ledge section. No sewer first 200 feet, old water and gas on opposite side. Sewer replacement included in blasting area. Will issue addendum addressing sewer profile and asbestos pipe. Test borings ended at Radio Circle but some ledge is likely to sta. 20+00. No separate item for repairing/reconnecting sewer services, it will be considered incidental to the pipe installation. Can use wye or inserta tee. Continue to Alec St. Flush, test, chlorinate, tie in services before connecting to the 14”. 6” valves on either side of the 14” do not work. Cut out 14”, install new tees, tie in 12”, turn 14” back on. Cut and cap 6” north, backfed from Montello. Continue north with new Install new 6” on Harkins to tie in to dead end. Some ledge near Montello. This section shall be hammered and paid as rock excavation as there are too many utilities to blast. Should only be about 1 foot or less to remove in this section. Connect all services and side streets before making connections at Montello intersection. Fuse connection to existing 2” PE at Montello. All costs associated with fusing shall be considered incidental to the 2” PE pipe. 4. Services: Contractor to do all taps. All services are new, connect existing copper where it crosses the new main or extend old service with coupling. Do not damage new curb and sidewalk. Separate item for tap and copper since some are just taps. Contractor shall take extra care when excavating services to avoid damaging the existing copper. 5. Side Streets: New gates are at gutter line. Side streets will need to be flushed and tested to the gate. Remove all old gate boxes. 6. Trench restoration/paving, asphalt escalator: The contractor is responsible for all trench restoration and paving. An asphalt escalator has been included in the bid. The project will be paved in sections as described in sequencing. 7. Record drawings (tie sheets): The City will provide a blank sheet for every house and the contractor needs to complete them for each service (corporation and coupling to existing service). 8. Traffic: Traffic is heavy for one hour in the morning and afternoon when school is in session. Due to school traffic and no easy detour routes, one lane of traffic needs to be maintained at all times. Short closures may be permitted for some crossings, with prior approval of the ---PAGE BREAK--- 3 City and traffic plan for each separate closure. A Traffic Plan will need to be submitted when getting the street opening and excavation permits at City Hall. There will be no fee for the street opening permit. 9. Testing: The Contractor is responsible for supplying chemicals, equipment and personnel for chlorinating, pressure testing, and delivering sample to lab. The City needs to be present to witness the pressure test. 10. Specs: Changes to the standard spec -time limit August 30, $500 inspection fee up from $200 -site maintenance/dust control-keep area clean and clear, access to homes. No sidecasting excavation, put directly into dump trucks -notice to property owners ahead of time when blocking access or shutting water -use dozer or grader to fine grade to ¼” tolerance as specified -use of private property- do not use lawns for tools, trench boxes or eating lunch. No parking or turning around in driveways. -blasting- review changes to 40.10, 8 inches below pipe 11. GAS- Unitil has special requirements when blasting within 10 feet such as installing matting to protect their pipe. Keep Unitil involved in the blasting plan. 12. Questions Are permit fees waived? Yes but still need to obtain street opening and excavation permits. Can material be stockpiled at the school or reservoir? Yes, we will talk to the school about securing an area for materials.
