This project requires a 10% bid bond. State/Federal Requirements: Davis Bacon, DBE, Buy America – the DBE Proposed Utilization form must be included with Bid (about page 44-46 of specs). DBE Attachment C must be included with every pay requisition, even if a DBE is not utilized during that time period. Only the bid forms need to be submitted, not the contract forms. Wage rates have increased – see spec documents(about page 50) – this project is using General Decision #ME120049, 1/6/12 – Highway. Time Limit: June 15, 2012 to September 15, 2012 - there is a current CSO project underway on Russell Street that is due to be complete June 1, 2012. Project Limits: Sabattus Street to Robinson Garden approximately 0.53 miles. Scope of Work: Milling 1.75” of pavement, shim roadway Hot Mix Asphalt, overlay with 1.25” of Hot Mix Asphalt. Work on this project must be done at night, between 7 p.m. and 7 a.m., M-F, due to the large volume of traffic that travels this road (14,000-20,000 vehicles per day). Any other times must be approved by the State and the City. The project includes curb-to-curb shim. There are a lot of manholes and catchbasins in the roadways. No catch basins need to be adjusted. There are sewer and storm manholes. The contractor will mill around the existing structures, those that are low and need to be adjusted will be adjusted by the respective utility, this can be determined during the pre-utility walk through. Lift rings will be used as much as possible to speed up the adjustment process. ---PAGE BREAK--- Item 202.2 Remove Pavement Surface shall be used on the streets including side streets, Item 202.203 Pavement Butt Joins will only be used in driveways that require overlaying, the intent is to match the existing driveways to the greatest extent practicable. Safety: The contractor shall be responsible to comply with all local, state, and federal regulations. Permits: The contractor shall be responsible for all permits including but not limited to: DIG SAFE, Excavation, Street Opening, Occupancy, Fill Sites – City issued permit fees will be waived. The contractor must go through the normal process for permits, but the City fees will be waived. Traffic Control: Contractor is responsible for traffic control; special requirements include, but are not limited to: Two VMBs required for the project (message boards). The Contractor shall be required to complete all work associated with the contract between the hours of 7:00 PM and 7:00 AM Monday through Friday. No work shall be completed at any other time unless previously approved by the Director. Utilities: Aerial: No Adjustments required. Underground: Contractor is to mill around the existing manholes, only those manholes which will not be able to work due to grade (i.e. are already low) will be adjusted. Milling within the streets shall be paid for as item 202.2 Removing Pavement Surface, Item 203.203 Pavement Butt Joints shall only be for driveways. Lewiston Department of Public Works (Sewer) – Will adjust necessary sewer manholes to grade after the structure is milled around. Lewiston Department of Public Works (Water) – Will lower gate boxes prior to milling and will raise gate boxes during paving operations. Contractor shall provide offsets to gate boxes prior to paving. Unitil- Will lower gate boxes prior to milling and will raise gate boxes during paving operations.
