The Finance Committee reserves the right to accept or reject any and all proposals. Please use the enclosed envelope or a clearly marked envelope with the bid name and number when submitting your bid. Only sealed bids will be accepted. Faxed bids will not be considered. Beauparlant Director of Budget/Purchasing NJB/syt ---PAGE BREAK--- City of Lewiston, Maine Department of Public Works Traffic Line Markings (Paint) Bid 2013-007 Bid Date: February 26, 2013 GENERAL: This work consists of furnishing and installing all necessary material to properly mark the longitudinal traffic lines for streets in the City of Lewiston. This includes center lines, edge lines and all lane dividing lines, including (splits or tails) at intersections. The successful bidder will provide all material, equipment and labor to install the traffic line markings. This includes but is not limited to paint, glass beads and any other incidental material required. The length of this contract will be for the next three years. MATERIALS & APPLICATIONS: All materials, preparation efforts and application of traffic lines shall be in accordance with the most recent revision of the State of Maine Department of Transportation’s “Standard Specifications” (Section 627 and 708.03). All pavement markings shall be applied in accordance with the U.S. Department of Transportation, Federal Highway Administration’s “Manual on Uniform Traffic Control Devices”. CLAIMS: The City of Lewiston will not be held responsible for any damages or injuries arising out of any efforts of the contractor. Any related claim will be referred to the Contractor. At his/her discretion, the contractor may wish to make personal restoration within a reasonable amount of time to the satisfaction of the individual filing the claim or process a claim with the contractor’s insurance carrier. INSURANCE: The Contractor shall furnish proof of adequate insurance coverage of the types and to the limits specified below naming the City of Lewiston as additional insured. Certificates of such insurance shall be filed with the Director of Budget and Purchasing within five days following notification of the Bid Award. WORKER’S COMPENSATION: Worker’s Compensation Insurance shall be provided by the Contractor for all employees employed at the site of the Project. If the Contractor hires a sub-contractor, the sub-contractor shall be required to provide Workers Compensation coverage for the latter’s employees unless such employees are covered by the protection afforded by the Contractor. All coverage shall be in accordance with State of Maine laws in effect and the requirements in the Industrial Accident Commission. GENERAL LIABILITY INSURANCE: General Liability Insurance with minimum limits of liability for bodily injury in the amount of $500,000, ---PAGE BREAK--- combined single limit and minimum limits of liability for property damage in the amount of $100,000 combined single limit. Collapse and Underground coverage will be required for any excavation activity. AUTOMOTIVE LIABILITY INSURANCE: Automotive Liability Insurance with minimum limits of liability for bodily injury in the amount of $500,000 for each occurrence and minimum limits of liability for property damage in the amount of $100,000. SUBSTANCE ABUSE: The Contractor shall be in compliance with the Federal Regulations pertaining to the mandatory alcohol and substance abuse testing, and must supply proof of compliance to the City upon the City’s request. The City of Lewiston, Department of Public Works reserves the right to suspend any operator who is suspected to be under the influence of alcohol and/or drugs. WORK SCHEDULING: The Contractor shall provide the Department of Public Works a schedule identifying when the work is planned to be done. The schedule should identify the streets planned to be done each day to allow department personnel to provide any notices required and perform inspections. This schedule will be submitted at least two weeks before work will begin for both the spring and fall schedules. Work will not begin until the schedule is received and approved. The department would strongly encourage the Contractor to schedule much of this work at night to avoid day time traffic. After contract award, the Department of Public Works will provide the successful bidder a map of the City identifying the locations of all streets if he/she requests. BILLING: The Contractor shall submit a bill in triplicate for work performed within 30 days after completing the work. Payment will be made along with other City accounts payable processed PAYMENTS: The accepted quantity of permanent pavement marking lines will be paid for at the unit contract price per linear foot. Appendix A to this contract provides an estimated total quantity of pavement marking lines. This is an estimated quantity and the contractor will track, document and submit with his/her bills the actual quantity of pavement marking lines installed on each street. additional one year periods (May 1, 2014 - November 1, 2014 and May 1, 2015 - November 1, 2015). The execution of these options is dependent upon funding availability and continued service requirements.
