Program Registration Policies Program Registration Policies Program Registration Policies Program Registration Policies ♣ Lewiston residents have first priority in all activities and programs. ♣ Office Hours: 8:00 am – 4:30 pm Monday through Friday. ♣ Registrations are on a first-come, first-serve basis. ♣ Fill out registration form, waiver agreement and sportsmanship agreements on site or or or or take from with you and mail with registration fee. ♣ Please make check payable to: City of Lewiston ♣ Mail to: 65 Central Ave., Lewiston, ME 04240 ♣ Credit card payments are accepted. ♣ Registration is acceptable only at specified sites. ♣ Registration will not be processed if form is incomplete or fee is not paid. ♣ Programs have a minimum and maximum participation number. ♣ There is a $25 charge for returned checks and a $6 charge for refunds. ♣ Non-resident fee of $15 for each program. ♣ A waiting list may be maintained if a program is filled. ♣ Participants must meet the age requirements of re3spective programs. ♣ Program news – The department staff will communicate directly before program start-up date. Please contact us if you have any questions. ♣ Program newsletters will be distributed on site by program directors. ♣ Program practices or games may be cancelled when school is closed due to weather or other reasons. Before and after-office hours, please call for the automated program cancellations, check local TV Stations, Web page or visit the blog site. We reserve the right to: reschedule, postpone or cancel activities, programs; change locations; or make other changes as required or necessary. Parents, please accompany youth to and from activities, programs, etc. We do not provide childcare during adult programs or meetings. Not responsible for lost or stolen items.
