CITY OF LEWISTON, MAINE ADVERTISEMENT FOR BIDS JEPSON BROOK CSO PHASE II BID 2012-004 DUE DATE: MARCH 27, 2012 @ 2:00 p.m. Sealed proposals, in envelopes plainly marked: “JEPSON BROOK CSO PHASE II” and addressed to: Norman J. Beauparlant, Director of Budget/Purchasing, City Hall, 27 Pine St., Lewiston, Maine 04240, will be received at the Purchasing Office until 2:00 p.m. on Tuesday, March 27, 2012, at which time and place bids will be opened and read aloud. A Mandatory Pre-Bid Conference will be held on Tuesday, March 13, 2012 at 10:00 A.M. at the Lewiston Public Works Facility, 103 Adams Avenue, Lewiston, in the Public Works 2 Floor Conference nd Room. All Prospective Bidders are required to attend. General Description of Work: The general scope of the project includes, but is not limited to, the following: The separation of combined sewer on Bellegarde Circle, Charles Street, Columbia Street, East Avenue, Farwell Street, Highland Avenue, Russell Street and Warren Avenue. The work shall be the complete and satisfactory furnishing and installing of approximately 410 LF of 8 inch dia. HDPE; 240 LF of 10 inch dia. HDPE; 2,650 LF of 12 inch dia. HDPE; 900 LF of 15 inch dia. HDPE; 1,230 LF of 18 inch dia. HDPE; 350 LF of 18 inch dia. HDPE; 50 LF of 36 inch dia. HDPE; 50 each of 4' dia. Catch basins; 16 each of 4' dia. Manholes; 2 each of 5' dia. Manholes; 2 each Type F Catch basin; 700 cubic yards of base aggregate and 4,200 cubic yards of sub-base aggregate; 8,500 square yards of fine grading of pipe trench areas; and all other incidental work as necessary to satisfactorily complete the project as outlined or implied in the Plans/Specifications All materials and labor required to complete the work will be supplied by the Contractor unless otherwise provided for in the Supplemental Specifications. The cost and expense of all the necessary labor, tools and equipment required to complete the work shall be included in the prices stated in the Proposal. A certified check or bank draft payable to the City of Lewiston or a satisfactory Bid Bond executed by the Bidder and a Surety Company in the amount equal to five percent of the Bid shall be submitted with each bid. No bid may be withdrawn for at least 60 days after receipt of bids unless released by the City of Lewiston. All inquiries concerning the request for proposal shall be directed to: Norman J.
