City of Lafayette Fire Department Fire Code Self-Inspection Guide Sheet _ Building Address: _ Legible from the street. _ All rear and side access doors should be addressed or labeled to indicate the space or business it serves. _ On-Site Fire Hydrants: _ No obstructions within 3’ of the sides and rear and unobstructed from the front. _ Clear height to bottom of large hose outlet of at least 1 foot. _ Hydrants subject to vehicular damage must properly protected from vehicles. _ Knox Box: _ Contains proper keys & applicable access codes for your business. _ Fire Protection Systems: _ Systems properly inspected & maintained: _ Fire sprinkler system _ Fire alarm system _ Fire standpipe system _ Commercial cooking hood system _ Fire department connection (FDC) on exterior of the building is unobstructed – protective caps or covers. _ Portable Fire Extinguishers: _ Minimum 5-pound A-B-C type for general-purpose areas. _ 75 feet maximum travel distance. _ K-rated extinguisher for commercial kitchen hood systems. _ Annual inspection required. _ 6-year maintenance for dry chemical or water extinguishers. _ 12-year hydrostatic test and maintenance for dry chemical or water extinguishers. _ 5-year hydrostatic test for carbon dioxide (CO2) extinguishers. _ Combustible Rubbish: _ Removed at least daily. _ Stored in approved containers located in approved locations. _ Rubbish Containers: _ Containers should not be located where subject to smoking materials or ignition sources. _ Dumpsters greater than 5-1/3 cubic feet provided with lids. _ Storage of Materials and Equipment: _ No storage in aisles or corridors. _ Maintained 2 feet below ceiling. _ Maintained 18 inches below sprinkler head deflectors. ---PAGE BREAK--- _ Combustible Storage: _ Must be orderly and not present a hazard to people. _ Storage is prohibited in: _ Boiler room _ Mechanical room _ Electrical equipment room _ Fueled Equipment: _ Storage prohibited within buildings, unless specially designed for that purpose. _ Means of Egress: ` _ Clear and unobstructed at all times. _ Path of exit travel clearly identified. _ Doors easily distinguishable as an exit door. _ Emergency power/lighting operational. _ Exit Door Locks: _ Operational and appropriate for exit type. _ Must be operable from inside without use of keys, special effort or special knowledge during normal business hours. _ Panic Hardware (if applicable): _ Operational and appropriately located on or about the door. _ Exit Lights: _ Operational and visible from means of egress. _ Illuminated when building is occupied. _ Electrical Hazards: _ Portable electric heaters should be 3 feet from combustible materials. _ All electrical cords should be in good condition with no abrasions, cuts, splits, or frays. Plug connections should not be altered or damaged. _ Extension cords should be used only with portable equipment. _ Electrical outlets shall have approved protective covers. _ Electrical control panels front area unobstructed for a minimum width of 30 inches and a height of 6 feet. _ Electrical panels breakers properly labeled. _ Breakers or protective blanks are required on all panels. _ Flammable and Combustible Liquids: _ No smoking in storage or use areas. _ Where permitted indoor storage shall not exceed 10 gallons. _ All storage shall be in approved flammable liquid storage containers. _ Hazardous Materials: _ All materials should be stored in approved containers. _ Storage areas should be identified by signs or placards. _ Contact the Lafayette Fire Prevention Bureau ([PHONE REDACTED], extension 3950) for chemical specific storage, use and handling requirements.
