Request for Qualifications Health Officer for Klickitat County Public Health Klickitat County is seeking Statements of Qualifications to enable the selection of an individual to serve as Klickitat County’s Public Health Departments Health Officer as it relates to the State of Washington statues and regulations. Compensation will be established by negotiation between the successful applicant and Klickitat County’s Board of Health. Demand for services will drive the compensation model. Options include establishing a competitive hourly rate or a fixed rate for providing services. PROPOSAL SUBMITTALS 1. Submit five copies (2 originals and 3 copies) of the proposal in time to be received by 12 noon, on Feb. 22, 2018. Late proposals will not be considered. 2. No faxed, emailed, or telephone proposals will be accepted. 4. All proposals must be in a sealed envelope and clearly marked “Proposal for Health Officer”. 5. Cost of preparation of proposals will be borne by the applicant 6. Klickitat County shall award the contract to the applicant whose proposal will best serve the interest of the County, taking into account, but not limited to, experience and expertise. 7. This request does not constitute an offer of employment or to contract for services. 8. The County reserves the option to retain all proposals, whether selected or rejected. Once submitted, the proposals and any supplemental documents become property of the Board of Health. 9. The County reserves the right to reject all proposals and re‐advertise the RFQ. 10. Selection will be made on the basis of the proposals as submitted. The Selection Committee members are not to be contacted by the proposers. PROPOSAL FORMAT Individuals wishing to be considered for selection will submit a packet that addresses the following: 1. Provide the following information on the cover page of your submittal: a. Legal name of applicant b. Street address c. City, State, Zip Code d. Daytime phone e. Email f. Provide signature of person submitting the response, with Title and Date 2. Each proposal shall include the following information, with each response labeled as indicated below: A. A resume and detailed list of acquired licenses and qualifications for Health Officer as required by RCW 70.05.050. B. Briefly tell us what experience you have had working in each of the following areas. If you have not had any experience, please tell us what course of action you would take or what resources you have available to you to ensure that the public peace, health and safety of Klickitat County residences’ would be assured. Please have the appropriate letter correspond with your response. ---PAGE BREAK--- a. The review of clinical information and chest radiographs of patients undergoing screening, diagnosis or treatment of tuberculosis. b. Providing diagnostic classification and submitting physician orders or prescriptions for treatment or monitoring of tuberculosis. c. Serving as medical director and ordering/prescribing physician for clinical activities. d. Reviewing and/or revising clinical protocols and recommendations to maintain consistency with applicable guidelines or standards of practice. e. Providing consultation and responding to requests from community based health care providers regarding the management of patients suspected of having or having been exposed to communicable diseases. f. Being involved with a public health emergency. g. Providing epidemiological oversight or medical direction in the surveillance, outbreak, investigations and control measures of a communicable disease. h. Determining whether or not any person is affected or is sick with a dangerous, contagious or infectious disease. i. Informing the public as to the causes, nature, and prevention of disease and disability and the preservation, promotion and improvement of health within a community. C. At least 3 professional references, including phone numbers and other contact information. Interviews for select candidates are expected to take place on or around the first week in March, 2018 with a view to having the position filled by mid‐March 2018. The committee will collaborate with the individual selected to develop a mutually acceptable plan of work. Interested parties shall submit qualification packages by 12 noon, on February 22, 2018.
