Contents Inspections 2 Five-Year Renewal 5 Stormwater Maintenance Permit Instructions ---PAGE BREAK--- P a g e I 2 Inspections Below are the steps to conduct and submit the annual Stormwater Maintenance Permit inspections. 1. Go to the City of Kalispell’s Stormwater Maintenance Permit website: 2. On the Stormwater Maintenance Permit website, click the link titled “Annual Inspection Form”. 3. The “Annual Inspection Form” link will navigate to an online map. Read the directions in the pop-up (copied below). To perform an inspection, search or navigate to the location of the stormwater control and click the diamond. The pop-up will have an "Annual Inspection" button. The form will auto-populate data based on the location selected. Please ensure the correct location is selected. 4. Close the pop-up by clicking the X in the upper right-hand corner of the box. ---PAGE BREAK--- P a g e I 3 5. Search by address or navigate to the location of the stormwater control. a. Search by address in the upper right-hand corner. b. To search by Stormwater Maintenance Permit number, click the down arrow on the search bar and select “Stormwater Maintenance Permits”. Then type the permit number and press enter. c. To navigate to a location, click and drag to move around the map. To zoom, scroll in and out. The + and – on the left side of the map can be used to zoom in and out as well. ---PAGE BREAK--- P a g e I 4 6. Once the correct location is found, click the colored diamond and a pop-up with a blue “Annual Inspection” button will appear. Click the “Annual Inspection” button to access the specific inspection form for that site. *Please ensure the correct location is selected because the inspection forms are location dependent. The forms will automatically populate the map location, permit number, and supply the correct type of inspection. At the top of the pop-up, the location or name should appear. Relevant and helpful documents will be in the pop-up and under attachments. 7. By clicking the “Annual Inspection” button, a site-specific inspection form will open. Complete the form and click submit. A successful submission satisfies the yearly annual inspection requirement. ---PAGE BREAK--- P a g e I 5 Five-Year Renewal Below are the steps to submit the required five-year Stormwater Maintenance Permit renewal. *Five-year permit renewals require a report certified by an engineer that the stormwater management practices are operating as designed. At a minimum, the report must include an inspection of the control comparing the as-built plan and operation and maintenance manual to determine if the control is operating as designed and if maintenance is required. If the control requires maintenance or other changes, the report shall contain mitigation measures required and a timeline to complete measures prior to the next required inspection. 1. The link “Stormwater Maintenance Permit Application & Renewal” will navigate to the City of Kalispell’s online permit program, Cityworks. If you have a Cityworks account, login. If you have not applied online before, you will need to create an account by clicking “Sign Up”. ---PAGE BREAK--- P a g e I 6 4. Once logged in, click the “Create Application” button at the top center of the page. 5. Next click “Stormwater Permits”. 6. Then select “Stormwater Maintenance Permit” and click “Begin Application”. 7. Fill out the application and submit. *Please note that five-year permit renewals require a report certified by an engineer that the stormwater management practices are operating as designed. An auto email will be sent once the application is complete to the emails listed in the people section requesting the engineering report.
