Electronic copies of the amended plat in AutoCAD, AutoCAD map, Land Desktop, Civil Design or CAD program. 2 copies of the surveyed amended plat: Please submit 1 electronic copy and 1 hard copy on 24” x 36” paper size (both drawn to scale), including: ● Vicinity Map, legal description, original and proposed property lines; ● Subdivision name, including “Amended” and its location by section, township, and range; reference by dimension and bearing to a section corner or quarter section corner; ● Location, dimensions, and areas of all proposed changes to the plat; ● Stamp, date, and signature from an Idaho licensed land surveyor; ● All information from the existing plat are to be included on the amended plat, including, but not limited to: o Date of plat, approximate true north point, scale, name and contact information of licensed land surveyor; o Location of water wells, streams, canals, irrigation laterals, private ditches, washes, lakes, or other water features; R-1 ---PAGE BREAK--- o Acreage and addresses for each lot; o Required setbacks of proposed lots; o All monuments erected, corners, and other points established and legends for metal monuments; o Notation and explanation of all easements. ● Signature blocks. Review & Approval Process for Vacation 1. Upon acceptance of a complete application, the application will be reviewed in accordance with Idaho Code Title 50-1306A and 50-1324. A public hearing before the City Council will be scheduled for the next available date. 2. The City will notice the project as a public hearing in accordance with Title 67, Chapter 65; Section 6509 of the Idaho State Code. This provision includes notification posted in the newspaper, within the property boundaries, and mailed notice to property owners within the land being considered and within three hundred (300) feet of the external boundaries of the land being considered. The applicant is responsible for all costs associated with the vacation processes. 3. The City Council will hold a public hearing and consider the plat vacation, after which the Council will make a final decision to approve, approve with modifications, or deny the plat vacation. 4. If approved, the plat must be recorded within six months after approval. Should any information or representation submitted in connection with this application form be incorrect or untrue, I understand that the City of Jerome may rescind any approval or sufficiency determination, or take other appropriate action. In addition, I understand that if this application is approved by the City Council but not recorded within six months of the date of said approval then this application shall be considered null and void.
