1 MCLEAN COUNTY NURSING HOME General Information For Admission to McLean County Nursing Home you will need the following: 1. A pre-screening by CCU (Case Coordination Unit) 2. A physician’s order for admission 3. A current history and physical a. If admitting from a hospital, the H & P will be current. b. If admitting from another nursing facility, the H & P has to have been within the last 6 months. c. If admitting from home the H & P has to be done within 3 days prior to admission. d. If admitting from out of town or changing physicians, the H & P must be done within 3 days prior to admission. 4. Results of TB skin test or one will be done in facility. 5. Written physicians orders with original signature. On or before the day of admission the facility will need a copy of your Social Security Card, Medicare Card, Insurance Cards, and bank statement(s) if you are on, or will be applying for public assistance. The facility will also need any advance directives you have, such as Living Will, Health Care Power of Attorne y, Financial Power of Attorney, Guardianship, etc. Each year the resident, with physician approval, may receive a flu vaccine injection. All personal clothing and items will be marked for identification Do not bring valuables, expensive jewelry, medications or electrical appliances. If you wish to bring furniture, it must be approved prior to admission. The facility prefers washable vinyl lounge chairs. If a cloth chair is brought into the facility and develops and odor, you are responsible for removing the chair from the facility or having it professionally cleaned. All laundry baskets and trash cans must meet fire safety guidelines. Talk to Social Services before bringing in these items. No pictures are to be hung without approval from maintenance or housekeeping. Any food kept in the resident’s room must be in a sealed container. ( A tin, plastic container with a lid, etc.) Any item that needs to be refrigerated must be consumed the day it is brought into the facility. There is not adequate refrigerator space to keep large quantities. A limited number of phone jacks are available. If you are interested, your name will be placed on the waiting list. The cost is $1.00 a day V.A. contracted nursing home for veterans eligible for nursing home care. McLean County Nursing Home is not responsible for damaged, lost or misplaced items. Revised 7/5/13
